【摘要】：目的探討呼吸肌鍛練聯(lián)合無(wú)創(chuàng)通氣的肺康復(fù)策略的臨床應(yīng)用價(jià)值。方法將2012年1月至2015年12月在廣州呼吸疾病研究所診斷明確的穩(wěn)定期慢性阻塞性肺疾病(慢阻肺)患者88例隨機(jī)分為聯(lián)合組、呼吸肌鍛煉組、無(wú)創(chuàng)組及氧療組并干預(yù)8周。評(píng)價(jià)生活質(zhì)量(SRI)、呼吸肌力(MIP、MEP)、呼吸困難(MRC)、運(yùn)動(dòng)耐量(6MWT)及肺功能。結(jié)果聯(lián)合組、鍛煉組及無(wú)創(chuàng)組SRI總分、6MWT、MRC均顯著改變(P均0.05),且聯(lián)合組△SRI、△6MWD、△MRC和鍛煉組、無(wú)創(chuàng)組間差異有統(tǒng)計(jì)學(xué)意義(P均0.05)。聯(lián)合組、鍛煉組MIP、MEP顯著性提高(P均0.05),且聯(lián)合組△MIP、△MEP與鍛煉組間差異有統(tǒng)計(jì)學(xué)意義(P0.05)。肺功能差異無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論呼吸肌鍛練聯(lián)合無(wú)創(chuàng)通氣康復(fù)策略臨床效果最優(yōu)。
[Abstract]:Objective to explore the clinical application value of pulmonary rehabilitation strategy of respiratory muscle training combined with noninvasive ventilation. Methods 88 patients with chronic obstructive pulmonary disease (COPD) diagnosed in Guangzhou respiratory disease institute from January 2012 to December 2015 were randomly divided into combined group, respiratory muscle exercise group, non invasive group and oxygen therapy group and intervention for 8 weeks. The quality of life (SRI), respiratory muscle strength (MIP, MEP), dyspnea (MRC), exercise tolerance (6MWT) and lung function. Results combined group, exercise group and non invasive group SRI total score, 6MWT, MRC were significantly changed (P 0.05), and the joint group Delta SRI, Delta 6MWD, Delta MRC and exercise groups were statistically significant (all 0.05). The joint group, exercise group, significant There was a significant difference in sex (P 0.05), and the difference between the joint group Delta MIP, Delta MEP and the exercise group was statistically significant (P0.05). There was no significant difference in pulmonary function (P 0.05). Conclusion the clinical effect of respiratory muscle training combined with non invasive ventilation was the best.
