【摘要】：目的探討影響醫(yī)院獲得性肺炎患者困難撤機(jī)的因素。方法回顧性分析鄭州人民醫(yī)院RICU2008年1月-2011年6月收治的127例行有創(chuàng)機(jī)械通氣治療的醫(yī)院獲得性肺炎患者的臨床資料,將患者分為簡(jiǎn)單撤機(jī)組和困難撤機(jī)組,觀察患者年齡、氧合指數(shù)、APACHEⅡ評(píng)分、血清前白蛋白、轉(zhuǎn)鐵蛋白、致病菌以及合并癥對(duì)呼吸機(jī)撤離的影響。結(jié)果共有34例患者發(fā)生呼吸機(jī)撤離困難,困難撤機(jī)組患者的年齡、APACHEⅡ評(píng)分明顯高于簡(jiǎn)單撤機(jī)組(P0.001);其氧合指數(shù)、血清前白蛋白、轉(zhuǎn)鐵蛋白含量明顯低于簡(jiǎn)單撤機(jī)組;其鮑曼不動(dòng)桿菌、銅綠假單胞菌、金黃色葡萄球菌感染率明顯高于簡(jiǎn)單撤機(jī)組(P0.005);且常合并較嚴(yán)重的基礎(chǔ)疾病。結(jié)論肺部感染控制不理想、營(yíng)養(yǎng)不良、心功能不全可能是影響醫(yī)院獲得性肺炎患者撤機(jī)困難的主要因素。
