應(yīng)用ROC曲線評價MIP-1α在煤工塵肺早期診斷中的價值

【摘要】:[目的]構(gòu)建受試者工作特征(ROC)曲線模型,應(yīng)用ROC曲線探討血清巨噬細(xì)胞炎癥蛋白-1α(MIP-1α)在煤工塵肺病早期診斷中的價值。[方法]選擇某煤業(yè)集團(tuán)公司經(jīng)職業(yè)健康檢查的、肺部有小陰影但不夠診斷為塵肺一期的180名接塵工人作為觀察組,180名肺部完全正常的接塵工人作為接塵組,109名煤工塵肺一期患者作為病例組,采集空腹靜脈血,使用酶聯(lián)免疫吸附法測定3組研究對象血清中趨化因子MIP-1α的表達(dá)水平,采用ROC曲線對MIP-1α篩檢煤工塵肺的作用進(jìn)行評價。[結(jié)果]經(jīng)協(xié)方差分析,控制年齡因素的影響后,病例組MIP-1α的表達(dá)水平均高于接塵組和觀察組,差異均有統(tǒng)計學(xué)意義(P0.05),觀察組MIP-1α的表達(dá)水平高于接塵組,差異有統(tǒng)計學(xué)意義(P0.05)。對接塵工齡分層后進(jìn)行分析,結(jié)果表明病例組在10~年、20~年和30~38年3個工齡段MIP-1α表達(dá)水平均高于觀察組和接塵組,差異均有統(tǒng)計學(xué)意義(P0.05),觀察組在10~年、20~年和30~38年3個工齡段MIP-1α表達(dá)水平均高于接塵組,差異均有統(tǒng)計學(xué)意義(P0.05);病例組、觀察組和接塵組MIP-1α表達(dá)水平隨著工齡的增加有增高趨勢。構(gòu)建ROC曲線,曲線下面積為0.874,最佳界值為940.85 ng/L,靈敏度為75.2%,特異度為90.0%,漏診率為24.8%、誤診率為10%、約登指數(shù)為65.2%、陽性似然比為7.52、陰性似然比為0.28、符合率為0.87、Kappa值為0.63、陽性預(yù)測值為69.49%、陰性預(yù)測值為92.31%,收益是36人。[結(jié)論]經(jīng)ROC曲線評價,MIP-1α作為篩檢指標(biāo)具有較高的真實(shí)性和可靠性,在煤工塵肺早期診斷中具有較高的參考價值。
[Abstract]:[objective] to establish the model of roc curve, and to explore the value of serum macrophage inflammatory protein (1 偽) MIP-1 偽) in the early diagnosis of coal workers' pneumoconiosis by using ROC curve. [methods] A coal industry group company was selected for occupational health examination. 180 pneumoconiosis workers who had small shadows in their lungs but were not diagnosed as pneumoconiosis at one stage were treated as observation group, 180 workers with completely normal lungs were treated as dust exposure group, 109 coal worker pneumoconiosis patients were selected as case group, and fasting venous blood was collected. Enzyme linked immunosorbent assay (Elisa) was used to determine the expression of chemokine MIP-1 偽 in the serum of three groups of subjects, and the effect of MIP-1 偽 screening for coal worker pneumoconiosis was evaluated by ROC curve. [results] after controlling the influence of age factors, the effect of MIP-1 偽 screening on pneumoconiosis of coal workers was evaluated by the method of enzyme linked immunosorbent assay. The expression level of MIP-1 偽 in the case group was higher than that in the dust exposure group and the observation group, the difference was statistically significant (P 0.05). The expression level of MIP-1 偽 in the observation group was higher than that in the dust exposure group, and the difference was statistically significant (P 0.05). The results showed that the expression levels of MIP-1 偽 in the three working age groups were higher than those in the observation group and the dust-exposed group in 10 ~ 20 ~ and 30 ~ 38 years, and the difference was statistically significant (P < 0.05). The MIP-1 偽 expression level in the observation group was higher than that in the dust exposure group in 10 ~ 20 ~ and 30 ~ 38 years. The expression level of MIP-1 偽 in the case group, observation group and dust-exposed group increased with the increase of length of service. The ROC curve was constructed. The area under the curve is 0.874, the best threshold is 940.85 ng / L, the sensitivity is 75.2, the specificity is 90.0, the missed diagnosis rate is 24.8um, the misdiagnosis rate is 10, the Yorden index is 65.2, the positive likelihood ratio is 7.52, the negative likelihood ratio is 0.28, the coincidence rate is 0.87 Kappa value is 0.63, the positive predictive value is 69.49, the negative predictive value is 69.49, the positive likelihood ratio is 7.52, the negative likelihood ratio is 0.28, the accuracy is 0.63, the positive predictive value is 69.49. The predicted value of sex is 92.31 and the profit is 36 people. [conclusion] the ROC curve evaluation of MIP-1 偽 as screening index has high reliability and authenticity. It has high reference value in the early diagnosis of coal worker's pneumoconiosis.
