【摘要】：目的探討經(jīng)直腸CEUS鑒別診斷直腸癌與直腸間質(zhì)瘤的價(jià)值。方法選取接受手術(shù)切除的直腸腫瘤患者44例,根據(jù)術(shù)前經(jīng)直腸CEUS檢查結(jié)果分為直腸癌組29例和直腸間質(zhì)瘤組15例,另設(shè)同種腫瘤周圍正常腸壁分別為對(duì)照1組和對(duì)照2組。觀察造影劑在直腸腫瘤患者的充盈灌注情況,并與各自相應(yīng)對(duì)照組比較。以對(duì)照組為參照,采用SonoLiver分析軟件繪制腫瘤與正常腸壁的時(shí)間強(qiáng)-度曲線(TIC),并對(duì)曲線形態(tài)和峰值強(qiáng)度(IMAX)、上升時(shí)間(RT)、達(dá)峰時(shí)間(TTP)及平均渡越時(shí)間(mTT)進(jìn)行比較。結(jié)果與各自相應(yīng)對(duì)照組比較,直腸癌組總體造影表現(xiàn)為"快進(jìn)快出"的高增強(qiáng)模式,直腸間質(zhì)瘤組表現(xiàn)為"慢進(jìn)慢出"的低增強(qiáng)模式。直腸癌組IMAX值高于對(duì)照1組[(122.42±15.99)%vs100%],而RT[(6.02±1.54)s vs(8.01±1.63)s]、TTP[(8.30±1.59)s vs(10.73±3.03)s]、mTT[(14.40±2.51)s vs(18.77±4.14)s]均小于對(duì)照1組(P均0.05);直腸間質(zhì)瘤組IMAX低于對(duì)照2組[(55.40±8.33)%vs 100%],而RT[(9.73±1.61)s vs(7.50±1.82)s]、TTP[(18.03±1.91)s vs(9.51±1.24)s]、mTT[(20.17±23.10)s vs(17.92±1.81)s]均大于對(duì)照2組(P均0.05)。結(jié)論直腸癌與直腸間質(zhì)瘤的CEUS表現(xiàn)存在明顯差異,結(jié)合TIC和造影參數(shù),有助于鑒別直腸癌和直腸間質(zhì)瘤。
[Abstract]:Objective to evaluate the value of transrectal CEUS in differential diagnosis of rectal cancer and rectal stromal tumor. Methods Forty-four patients with rectal tumor underwent surgical resection were divided into rectal cancer group (n = 29) and rectal stromal tumor group (n = 15) according to the results of preoperative transrectal CEUS examination. The normal intestinal wall around the same tumor was divided into control group 1 and control group 2 respectively. The filling and perfusion of contrast media in rectal tumor patients were observed and compared with the corresponding control groups. SonoLiver analysis software was used to draw the time-intensity curve (TIC), of tumor and normal intestinal wall, and the curve shape and peak intensity (IMAX), rising time (RT), were compared with the control group. The peak time (TTP) was compared with the average transit time (mTT). Results compared with the corresponding control group, the overall enhancement pattern of rectal cancer group was "fast in and out" mode, and that of rectal stromal tumor group was "slow in and slow out" mode. The IMAX value of rectal cancer group was higher than that of control group [(122.42 鹵15.99)% vs100%], but RT [(6.02 鹵1.54) s vs () 8.01 鹵1.63) s, TTP [(8.30 鹵1.59) s vs () 10.73 鹵3.03) s]. MTT [(14.40 鹵2.51) s vs (18.77 鹵4.14) s] was lower than that of the control group (all P 0.05). The IMAX of rectal stromal tumor group was lower than that of control group [(55.40 鹵8.33)% vs 100%], but RT [(9.73 鹵1.61) s vs (7.50 鹵1.82) s], TTP [(18.03 鹵1.91) s vs (9.51 鹵1.24 s]. MTT [(20.17 鹵23.10) s vs () 17.92 鹵1.81 s] was higher than that of the control group (P 0.05). Conclusion there is significant difference in CEUS between rectal cancer and rectal stromal tumor. Combined with TIC and imaging parameters, it is helpful to distinguish rectal cancer from rectal stromal tumor.
