正常腦老化中腦白質(zhì)改變的MRI研究

【摘要】:目的:通過VBM技術(shù)研究正常腦老化過程中白質(zhì)體積發(fā)生年齡相關(guān)性改變的規(guī)律及性別差異性。 方法:自2013年8月-2013年11月招募健康成年志愿者,均行常規(guī)顱腦核磁共振成像(magnetic resonance imaging, MRI)和T1加權(quán)3D快速場(chǎng)回波(T1weighted image Fast Field Echo, T1-FFE)序列檢查。共選出187例符合納入標(biāo)準(zhǔn)的受檢者,其中男性92例,女性95例,年齡范圍20-78歲,平均年齡47.2歲,中位年齡47歲。將所有被試按性別和年齡分為青年組、中年組和老年組。采用基于體素的形態(tài)測(cè)量法(Voxel-basedmorphometry, VBM),對(duì)腦結(jié)構(gòu)圖像T1-FFE序列進(jìn)行預(yù)處理,分割平滑后得到全腦白質(zhì)體積數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。采用兩樣本t檢驗(yàn)比較男女性別之間全腦白質(zhì)體積有無差異。采用方差分析方法對(duì)不同性別、年齡組間白質(zhì)體積的變化進(jìn)行分析。采用回歸分析檢測(cè)所有研究對(duì)象全腦白質(zhì)體積與年齡相關(guān)性進(jìn)行分析。視P0.05的腦區(qū)為差異有統(tǒng)計(jì)學(xué)意義。同時(shí)采用回歸分析檢測(cè)各感興趣區(qū)白質(zhì)體積與年齡的相關(guān)關(guān)系。 結(jié)果: 1全腦白質(zhì)體積性別差異分析:調(diào)整顱內(nèi)總體積后,,女性較男性具有更大的白質(zhì)體積,差別具有統(tǒng)計(jì)學(xué)意義(t=3.79, P=0.0001, FDR校正)。局部白質(zhì)體積在女性較大的腦區(qū)主要位于:延髓、腦橋、兩側(cè)大腦腳、兩側(cè)顳葉內(nèi)側(cè)回、兩側(cè)小腦前葉、左側(cè)枕葉、兩側(cè)額上回、左側(cè)額葉眶回、兩側(cè)頂葉、扣帶回膝部。局部白質(zhì)體積在男性較大的區(qū)域主要位于兩側(cè)小腦后葉、兩側(cè)島葉外囊、兩側(cè)額葉直回,以及在兩側(cè)頂葉和左側(cè)枕葉局部腦區(qū)。 2不同年齡組之間白質(zhì)體積改變的分析:使用雙因素方差分析分別比較三組之間的腦白質(zhì)體積差異,發(fā)現(xiàn)每?jī)山M之間白質(zhì)體積均存在顯著的年齡主效應(yīng)(P0.05, FDR校正)。中年組與青年組比較,白質(zhì)體積表現(xiàn)為增加的腦區(qū)位于兩側(cè)額葉直回、兩側(cè)中央前回、兩側(cè)放射冠、兩側(cè)內(nèi)囊前肢、右側(cè)外囊、右側(cè)小腦中腳、兩側(cè)顳葉鉤回、兩側(cè)豆?fàn)詈、右?cè)枕葉。老年組和青年組比較顯示局部白質(zhì)體積減少的腦區(qū)主要位于兩側(cè)額葉、兩側(cè)大腦腳、兩側(cè)內(nèi)囊后肢、左側(cè)內(nèi)囊前肢、胼胝體壓部、兩側(cè)枕葉、左側(cè)顳葉、前扣帶回。老年組和中年組比較顯示局部白質(zhì)體積萎縮的腦區(qū)主要位于兩側(cè)顳葉內(nèi)側(cè)回、兩側(cè)顳下回、左側(cè)顳上回、兩側(cè)大腦腳、兩側(cè)丘腦、兩側(cè)額中回、左側(cè)額下回、兩側(cè)放射冠、胼胝體體部和膝部、兩側(cè)扣帶回、左側(cè)頂葉、兩側(cè)海馬旁回、右側(cè)楔前葉及兩側(cè)小腦部分腦區(qū)。另外,青年組和中年組比較時(shí),若不采用多重對(duì)比校正(P0.001,未校正),部分腦區(qū)表現(xiàn)為女性的相對(duì)保護(hù),主要位于兩側(cè)小腦半球、腦橋前部、延髓、左側(cè)放射冠、左側(cè)枕葉等。同樣老年組和青年組比較時(shí),若不采用多重對(duì)比校正(P0.001,未校正)可觀察到部分腦區(qū),如兩側(cè)放射冠、兩側(cè)海馬、左側(cè)外囊、左側(cè)內(nèi)囊后肢等,表現(xiàn)為女性對(duì)上述腦區(qū)的相對(duì)保護(hù)作用。青年組、中年組及中年組三組中各兩組之間均未發(fā)現(xiàn)顯著的年齡×性別交互效應(yīng)(P0.05, FDR校正)。 3腦白質(zhì)體積與年齡的相關(guān)性分析:全腦白質(zhì)體積與年齡呈非線性、二次方式倒“U”型變化趨勢(shì),R2=0.124, P<0.000。將各感興趣區(qū)白質(zhì)體積作為應(yīng)變量進(jìn)行回歸分析,結(jié)果顯示額葉為年齡相關(guān)性最強(qiáng)的腦區(qū),R2=0.13,P0.001。而枕葉出現(xiàn)年齡相關(guān)性萎縮的時(shí)間最晚。兩側(cè)小腦半球、中腦、腦橋及延髓與年齡間未見顯著相關(guān)性。 結(jié)論: 1在調(diào)整顱內(nèi)總體積的情況下,全腦白質(zhì)體積分布存在性別的差異,在大部分腦區(qū)女性具有更大白質(zhì)體積,并且在老化過程中,部分腦區(qū)(如兩側(cè)放射冠、兩側(cè)海馬旁回、左側(cè)枕葉、左側(cè)大腦腳及左側(cè)內(nèi)囊等)女性表現(xiàn)為相對(duì)保護(hù)作用。 2在正常老化過程中,全腦白質(zhì)體積與年齡呈現(xiàn)為二次方式、倒“U”型相關(guān)性改變,約在50歲達(dá)到最大值,之后逐漸減少。但是不同腦區(qū)一般以“后進(jìn)先出”的規(guī)律呈現(xiàn)不同模式,額葉年齡相關(guān)性最強(qiáng),枕葉受年齡影響時(shí)間較晚。 3兩側(cè)小腦半球、中腦、腦橋及延髓隨年齡并未表現(xiàn)出顯著相關(guān)性。
[Abstract]:Objective : To study the changes of age - related changes in white matter volume during normal brain aging by VBM technique .
Methods : Healthy adult volunteers were recruited from August 2013 to November 2013 . 187 cases were selected according to normal brain nuclear magnetic resonance imaging ( MRI ) and T1 weighted 3D fast field echo ( T1 - FFE ) sequence .
Results :
1 Whole brain white matter volume sex difference analysis : after adjusting the total volume of intracranial volume , the female had greater white matter volume than the male , and the difference had statistical significance ( t = 3.79 , P = 0.0001 ) . The local white matter volume is mainly located in the medulla , the brain bridge , the cerebral feet on both sides , the medial gyrus of the temporal lobe on both sides , the frontal lobe of the left frontal lobe , the left occipital lobe , the frontal gyrus of the left frontal lobe , the left frontal lobe , the parietal lobe on both sides , and the back knee .
2 . Analysis of the change of white matter volume between different age groups : A two - factor variance analysis was used to compare the difference of brain white matter volume between the three groups . It was found that there was a significant age - dominant effect between the two groups ( P0.05 ) . In the aged group and the middle - aged group , the brain areas with reduced local white matter volume were mainly located on the medial gyrus , the left parietal lobe , the left superior cerebellar peduncle , the left temporal lobe , the left occipital lobe , the right occipital lobe , the left temporal lobe and the left occipital lobe . The relative protective effects of female on the above - mentioned brain regions were observed in the left inner capsule hind limbs , etc . There were no significant differences in age and sex between the young group , middle - aged group and middle - aged group ( P0.05 ) .
The relationship between brain white matter volume and age was analyzed : the whole brain white matter volume and age were non - linear , the quadratic form inverted " U " type change trend , R2 = 0.124 , P < 0.000 . The white matter volume of each region of interest was analyzed by regression analysis . The results showed that the frontal lobe was the most age - related brain region , R2 = 0.13 , P0.001 .
Conclusion :
1 . In the case of adjusting the total intracranial volume , there is a gender difference in the whole brain white matter volume distribution , and in most brain regions , women have a greater white matter volume , and in the aging process , women in some brain regions ( such as bilateral radiation crowns , parahippocampal gyrus , left occipital lobe , left brain foot , left side inner capsule , etc . ) behave as a relative protection function .
2 . In the course of normal aging , the whole brain white matter volume and age were presented in the second way , the " U " type correlation was changed , the maximum was reached at the age of 50 years , and then decreased gradually . However , the different brain regions generally showed different patterns with the laws of " LIFO " , the correlation between frontal and leaf age was strongest , and the age of the occipital lobe was relatively late .
There was no significant correlation between the cerebellum hemisphere , midbrain , brain bridge and medulla with age on both sides .
