【摘要】：目的探討甲狀腺峽部乳頭狀癌(PTC)聲像圖表現(xiàn)。方法回顧性分析103例峽部及270例側(cè)葉PTC患者,根據(jù)結(jié)節(jié)位置分組:I_1組(PTC僅位于峽部)、I_2組(伴側(cè)葉PTC的峽部PTC)、L_1組(PTC僅位于側(cè)葉)和L2組(I2組患者的側(cè)葉PTC)。結(jié)果 I_1組和I_2組超聲診斷符合率為79.9%和83.3%低于L_1組91.7%。與L_1組相比,I_1組PTC多表現(xiàn)為邊緣規(guī)則、縱橫比1、回聲均勻、甲狀腺背景回聲不均勻,I_1組結(jié)節(jié)接觸和突破被膜比例、病理證實侵犯被膜比例、合并頸部淋巴結(jié)轉(zhuǎn)移率也更高,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論峽部PTC超聲表現(xiàn)與側(cè)葉PTC有所不同,結(jié)節(jié)與被膜關(guān)系更緊密,易發(fā)生淋巴結(jié)轉(zhuǎn)移。
[Abstract]:Objective to investigate the ultrasonographic features of thyroid isthmus papillary carcinoma (PTC). Methods 103 cases of isthmus and 270 cases of lateral lobe PTC were analyzed retrospectively. Group I_1 (PTC only in isthmus), I_2 group (PTC isthmus PTC), L_1 group (PTC only in lateral lobe) and L2 group (lateral lobe PTC) in I2 group. The coincidence rate of 79.9% and 83.3% was lower than that of group 91.7%. in group L_1 and group L_1, PTC in group I_1 showed marginal rule, the ratio of vertical and horizontal ratio was 1, the echo was uniform, the echo of thyroid background was unevenly, the proportion of nodule contact and breakthrough membrane in group I_1, pathology confirmed the proportion of invasion of the membrane, and the rate of cervical lymph node metastasis was also higher, the difference was statistically significant (P0.05). The PTC findings of isthmus are different from those of lateral lobe PTC. The nodules are more closely related to the capsule and are prone to lymph node metastasis.
