【摘要】：【目的】探討超聲造影(CEUS)在困難部位惡性腫瘤放射性粒子植入治療中的應(yīng)用價(jià)值�！痉椒ā砍曉煊耙龑�(dǎo)下對(duì)24例共31個(gè)困難部位(腫瘤邊緣與膽囊、肝門部、胃腸、左右門靜脈主干或三支肝靜脈、下腔靜脈距離不足0.5 cm)的惡性腫瘤行放射性碘粒子植入治療術(shù)。分別于術(shù)前和術(shù)后1、3、6、12月進(jìn)行增強(qiáng)CT及超聲造影隨訪。根據(jù)RECIST判定標(biāo)準(zhǔn)評(píng)價(jià)局部療效,根據(jù)超聲造影早期增強(qiáng)程度評(píng)估病灶血供變化�！窘Y(jié)果】平均隨訪時(shí)間11個(gè)月,范圍1~46個(gè)月。增強(qiáng)CT顯示術(shù)后1、3、6、12月總有效率分別為0%、7.7%、21.1%及54.6%。超聲造影顯示術(shù)后1、3、6、12月富血供病灶所占比例分別為89.7%、50.0%、36.8%及27.3%。無嚴(yán)重并發(fā)癥及粒子植入相關(guān)死亡發(fā)生�！窘Y(jié)論】超聲造影在困難部位惡性腫瘤放射性粒子植入治療中能安全有效地引導(dǎo)放射性粒子的植入,對(duì)腫瘤局部療效的評(píng)判也具有一定的幫助。
[Abstract]:[objective] to evaluate the value of contrast-enhanced ultrasound (CEUs) in the treatment of malignant tumors with radioactive seeds implantation in difficult areas. [methods] Twenty-four patients with 31 difficult sites (tumor edge and gallbladder, hilar hepatis, gastrointestinal tract) were treated by contrast-enhanced ultrasound. Main portal vein or three hepatic veins, Radioactive iodine seed implantation was performed on malignant tumors with inferior vena cava (less than 0. 5 cm). Enhanced CT and contrast-enhanced ultrasonography were performed before and after operation, respectively, on December. The local efficacy was evaluated according to RECIST criteria. The changes of blood supply were evaluated according to the early enhancement of contrast-enhanced ultrasound. [results] the mean follow-up time was 11 months. The range was 1 ~ 46 months. The total effective rate of CT was 0 and 7.721% and 54.6%, respectively. Contrast-enhanced CT showed that the proportion of lesions with rich blood supply in December was 89.70.30% and 27.33.Conclusion there were no serious complications and death associated with seed implantation. Contrast-enhanced ultrasound can guide the implantation of radioactive particles safely and effectively in the treatment of malignant tumor in difficult position. It is also helpful to evaluate the local curative effect of tumor.
