        目的本研究的目的是評(píng)價(jià)錐形束CT與螺旋CT對(duì)髁突成像三維重建后線性測(cè)量的準(zhǔn)確性和可靠性。材料和方法選取10個(gè)干燥的完整的40-60歲的成年男性的下頜骨,分別采用意大利維羅納Quantitative Radiology公司生產(chǎn)的Newtom3G CBCT機(jī)和美國(guó)通用電器公司生產(chǎn)Light Speed16層螺旋CT機(jī)對(duì)其進(jìn)行掃描,然后將獲得的CT數(shù)據(jù)導(dǎo)入Mimics10.01軟件,三維重建其相應(yīng)的三維數(shù)字化模型。本研究選取12個(gè)測(cè)量項(xiàng)目對(duì)髁突進(jìn)行了系統(tǒng)的測(cè)量。兩名研究員在2周內(nèi)分別對(duì)錐形束CT數(shù)字化模型、螺旋CT數(shù)字化模型和髁突實(shí)體進(jìn)行3次測(cè)量,并將6次測(cè)量的平均值分別作為錐形束CT測(cè)量值、螺旋CT測(cè)量值、實(shí)體測(cè)量值。采用SPSS13.0統(tǒng)計(jì)軟件分別對(duì)錐形束CT測(cè)量數(shù)據(jù)與實(shí)體測(cè)量值、螺旋CT測(cè)量值與實(shí)體測(cè)量值、錐形束CT測(cè)量數(shù)據(jù)與螺旋CT測(cè)量值進(jìn)行統(tǒng)計(jì)分析,以P≤0.05為檢驗(yàn)水準(zhǔn),用配對(duì)t檢驗(yàn),比較不同測(cè)量方法之間的一致性。測(cè)量的準(zhǔn)確性用平均絕對(duì)誤差和平均絕對(duì)百分誤差評(píng)價(jià)。以P≤0.05為檢驗(yàn)水準(zhǔn),用配對(duì)t檢驗(yàn)的方法比較錐形束CT和螺旋CT對(duì)髁突三維重建后線性測(cè)量的準(zhǔn)確性。用兩因素方差分析對(duì)研究員自身和研究員之間的測(cè)量值進(jìn)行統(tǒng)計(jì),研究員自身和研究員之間測(cè)量的可靠性用組內(nèi)相關(guān)系數(shù)來(lái)評(píng)價(jià)。結(jié)果1.三種測(cè)量方法所測(cè)得的數(shù)據(jù)相互之間無(wú)明顯統(tǒng)計(jì)學(xué)差異(P>0.05)。錐形束CT對(duì)髁突成像三維重建后的線性測(cè)量與螺旋CT對(duì)髁突成像三維重建后的線性測(cè)量有一致性。2.錐形束CT對(duì)髁突三維重建后線性測(cè)量的準(zhǔn)確性與螺旋CT對(duì)髁突三維重建后線性測(cè)量的準(zhǔn)確性之間無(wú)統(tǒng)計(jì)學(xué)差異。錐形束CT的平均絕對(duì)誤差為0.27mm(±0.19mm),螺旋CT的平均絕對(duì)誤差為0.20mm(±0.17mm)。錐形束CT和螺旋CT的平均絕對(duì)百分誤差分別為0.98%(±0.73%)和1.26%(±1.50%)。3.錐形束CT和螺旋CT的研究員自身和研究員之間測(cè)量值的組內(nèi)相關(guān)系數(shù)均大于0.9,有較好的測(cè)量可靠性結(jié)論錐形束CT測(cè)量的準(zhǔn)確性雖然比螺旋CT三維測(cè)量的準(zhǔn)確性略小,但仍具有較好的準(zhǔn)確性和可靠性,可以作為一種對(duì)患者損傷小、能準(zhǔn)確、便捷獲得三維圖像的影像學(xué)方法,替代螺旋CT用于髁突的臨床檢查和研究。

    ObjectiveThe purpose of this study was to evaluate the accuracy and reliability of condylar three-dimensional reconstruction linear measurements obtained from cone beam CT (CBCT) and multislice CT (MSCT)Materials and Methods10dry male mandibles aged form40-60years old were selected. The mandibles were scanned with NewTom3G CBCT(Quantitative Radiology, Verona. Italy) and Light Speedl6MSCT(General Electric, America). Mimics10.01software was used for three-dimensional reconstruction and measurements on imaging data. Twelve interlandmark linear measurements were made directly on the condyle and compared with the same measurements made on the CBCT scans and MSCT scans. All measurements were made by2operators on3separate occasions. The CT measurements were established as the mean of the6measurements. Anatomic truth was established with6direct caliper measurements. Analysis of the data was conducted by using SPSS13.0statistical software. The consistency among the three measurements was analyzed by paired t test. A P value of P≤0.05was used to assign statistical significance. The accuracy of these measurements was expressed by means of the absolute error (MAE) and absolute percentage error (APE).As a measure of reliability, the intraclass correlation coefficient (ICC) for absolute agreement based on a2-way random-effects analysis of variance (ANOVA) was calculated among the3measurement methods.Results1.No significant statistical differences among the three measurement methods(P>0.05). Linear measurements of condylar three-dimensional reconstruction obtained from CBCT were consistent with MSCT.2. No significant statistical differences between the measurement accuracy of CBCT and MSCT.MAE was0.27mm (±0.19mm) for the CBCT device and0.20mm (±0.17mm) for the MSCT device. APE was0.98%(±0.73%) and1.26%(±1.50%), respectively.3. The ICC of the measurements of the images of CBCT and MSCT were all more than0.9. The reliability of condylar three-dimensional reconstruction linear measurements obtained from cone beam CT and multislice CT was good.ConclusionMSCT scans proved slightly more accurate in linear measurement. The accuracy and the reliability of CBCT were good. CBCT device can be used in condylar clinical examination instead of MSCT device.
