【摘要】：目的分析艾滋病病毒(HIV)抗體陽(yáng)性與陰性暗娼的高危行為,為開(kāi)展針對(duì)性的干預(yù)工作提供科學(xué)的依據(jù)。方法以2012-2013年暗娼哨點(diǎn)監(jiān)測(cè)中發(fā)現(xiàn)的既往已報(bào)告的HIV感染者作為HIV抗體陽(yáng)性組,按1∶2配比選擇HIV抗體陰性者作為陰性組。比較兩組間在人口學(xué)特征、高危行為、安全套使用率及接受干預(yù)服務(wù)等差異。結(jié)果 2012-2013年暗娼哨點(diǎn)共發(fā)現(xiàn)既往HIV感染者224例,共匹配205對(duì)。HIV抗體陽(yáng)性組最近一個(gè)月與客人性行為時(shí)堅(jiān)持使用安全套的比例為88.8%,HIV抗體陰性組為80.7%,粗比值比(OR)值為2.273,95%可信區(qū)間(CI):1.271~4.066,調(diào)整后OR值為2.178,95%CI:1.053~4.505;既往HIV陽(yáng)性暗娼亞組分析中,除艾滋病知識(shí)知曉率(報(bào)告年限1年以上組高于1年以內(nèi)組,P=0.036),其余各因素在是否接受抗病毒治療和報(bào)告年限亞組中差異均無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論 HIV陽(yáng)性暗娼在知曉陽(yáng)性結(jié)果后,高危行為有所改進(jìn),但仍存在感染嫖客的危險(xiǎn),且干預(yù)效果不受報(bào)告年限影響。常規(guī)干預(yù)措施對(duì)該人群效果不明顯,應(yīng)積極探索早期抗病毒治療等有效干預(yù)手段,借助社區(qū)組織等感染者小組干預(yù)模式,進(jìn)一步加強(qiáng)對(duì)陽(yáng)性暗娼的管理。
[Abstract]:Objective to analyze the high risk behavior of HIV (HIV) antibody positive and negative female prostitutes, and to provide scientific basis for targeted intervention. Methods the previously reported HIV infection found in the sentinel surveillance of female prostitutes from 2012 to 2013 was used as the HIV antibody positive group, and the HIV antibody negative group was selected as the negative group according to the proportion of 1:2. The demographic characteristics, high risk behavior, condom use rate and intervention service were compared between the two groups. Results from 2012 to 2013, 224 cases of previous HIV infection were found in the sentinel of female prostitutes, and a total of 205 pairs were matched. The proportion of patients with positive HIV antibody who insisted on using condoms during sex with guests in the last month was 88.8%, and that of HIV antibody negative group was 80.7%.
