【摘要】：目的分析我市實施全球基金耐多藥結核病防治項目工作效果,為今后制定耐多藥結核病控制策略提供依據(jù)。方法對2012年4月至2014年6月期間,我市全球基金結核病項目報表、實驗室登記本、耐多藥肺結核病案等資料進行分析。結果全球基金項目期間,我市共篩查耐多藥可疑者1 081例,篩查率86.07%,耐多藥肺結核檢出率6.30%,廣泛耐多藥檢出率0.25%,新患者耐多藥檢出率顯著低于其他不同類別的涂陽病人的檢出率(χ2=81.69,P0.01)。確診52例耐多藥病人,患者納入治療率78.85%,治療6個月時,痰涂片陰轉(zhuǎn)率66.67%,痰培養(yǎng)陰轉(zhuǎn)率63.33%。治療滿12個月時,痰涂片及培養(yǎng)陰轉(zhuǎn)率均為54.17%。結論通過全球基金項目的實施,我市建立了"地級市診療,縣區(qū)級篩查,社區(qū)督導服藥"的耐多藥結核病防治新模式,耐多藥防治工作得到加強和提高,為今后防治工作提供了良好的指導和借鑒。
[Abstract]:Objective to analyze the effect of global fund multi-drug resistance TB control project in our city, and to provide basis for formulating control strategy of MDR-TB in the future. Methods from April 2012 to June 2014, the data of TB project report, laboratory register and multi-drug resistant pulmonary tuberculosis (MDR-TB) were analyzed. Results during the period of the Global Fund Project, 1 081 cases of suspicious multidrug resistance were screened in our city, the screening rate was 86.07, the detection rate of multidrug resistance tuberculosis was 6.30, and the detection rate of extensive multidrug resistance was 0.25%. The detection rate of multidrug resistance in new patients was significantly lower than that in other smear positive patients (蠂 2 / 81.69 / P0.01). 52 patients with multidrug resistance were diagnosed, and the rate of treatment was 78.85. After 6 months of treatment, the sputum smear negative conversion rate was 66.67 and the sputum culture negative conversion rate was 63.33. After 12 months of treatment, the sputum smear and culture negative rate were 54.17%. Conclusion through the implementation of the Global Fund project, we have established a new model of multi-drug resistant tuberculosis prevention and treatment, which is called "prefectural level, county level screening, community supervision taking medicine", and the prevention and treatment of multi-drug resistance has been strengthened and improved. It provides good guidance and reference for the prevention and cure work in the future.
