【摘要】：目的分析布魯桿菌病非流行區(qū)以不明原因發(fā)熱為首發(fā)癥狀就診并確診的布魯桿菌病患者的不典型臨床表現(xiàn),提高對非流行區(qū)不典型布魯桿菌病的臨床認識。方法收集2013年1月——2016年1月北京醫(yī)院以不明原因發(fā)熱為首發(fā)癥狀就診后經(jīng)布魯桿菌血培養(yǎng)檢查陽性確診的布魯桿菌病患者10例的臨床資料,進行回顧性分析,并結(jié)合相關(guān)文獻復(fù)習(xí)。結(jié)果 10例布魯桿菌病患者,其中男7例,女3例;診斷時年齡40~70歲,平均年齡56.3歲。均來自非牧區(qū)。3例有養(yǎng)羊或羊接觸史,3例有進食牛羊肉史,1例有在家加工生羊肉史,3例無明確流行病學(xué)接觸史。10例均以發(fā)熱伴或不伴其他癥狀就診。首診科室包括急診科3例,骨科2例,呼吸科2例,風(fēng)濕科1例,神經(jīng)內(nèi)科1例,血液科1例。10例患者布魯桿菌血培養(yǎng)及凝集試驗均為陽性。結(jié)論布魯桿菌病非流行區(qū)對于不明原因發(fā)熱的患者應(yīng)重視散發(fā)布魯桿菌病的可能,反復(fù)血培養(yǎng)為最終確診提供依據(jù)。
[Abstract]:Objective to analyze the atypical clinical manifestations of patients with brucellosis in non-endemic areas with unknown fever as the first symptom, and to improve the clinical understanding of atypical brucellosis in non-endemic areas. Methods from January 2013 to January 2016, the clinical data of 10 patients with brucellosis diagnosed by brucellae blood culture test were collected and analyzed retrospectively. Combined with the review of relevant literature. Results Ten patients with brucellosis, including 7 males and 3 females, were diagnosed at the age of 4070 years with an average age of 56.3 years. All of them were from non-pastoral areas. 3 cases had a history of sheep raising or contact, 3 cases had a history of feeding beef and mutton, 1 case had a history of processing raw mutton at home, 3 cases had no definite history of epidemiological contact, and 10 cases were treated with fever or without other symptoms. The first department included emergency department (3 cases), orthopaedics (2 cases), respiratory department (2 cases), rheumatology (1 case), neurology (1 case) and hematology (1 case). The blood culture and agglutination test of brucellosis were positive in 10 cases. Conclusion in non-endemic areas of brucellosis, the possibility of sporadic brucellosis should be paid more attention to for patients with fever of unknown origin, and repeated blood culture may provide basis for final diagnosis.
