【摘要】：目的通過(guò)對(duì)有輸血史的惡性瘧疾感染者進(jìn)行檢測(cè)和溯源分析,確定其傳染源和傳播方式。方法對(duì)患者和為其輸血的8名獻(xiàn)血者血樣進(jìn)行瘧疾快速檢測(cè)、血涂片鏡檢和Real-time PCR檢測(cè),并對(duì)陽(yáng)性血樣進(jìn)行分子溯源和基因分型。結(jié)果患者與1名非洲返鄉(xiāng)獻(xiàn)血者在瘧疾快速診斷試劑檢測(cè)、血涂片鏡檢和Real-time PCR 3項(xiàng)檢測(cè)中,均顯示為惡性瘧陽(yáng)性。兩者血樣SSU rRNA基因序列兩兩比對(duì)后同源性為100%,Pf EMP-1基因分型顯示受血者和獻(xiàn)血者血樣均為惡性瘧原蟲(chóng)K1型和MD20型混合感染。確定該患者的惡性瘧確因輸入非洲返鄉(xiāng)獻(xiàn)血者的血液而感染。討論由于我國(guó)未將瘧疾篩查列入獻(xiàn)血前檢測(cè),因此境外返鄉(xiāng)人員在瘧疾潛伏期或復(fù)發(fā)期內(nèi)參加獻(xiàn)血易引起輸血傳播瘧疾。采供血機(jī)構(gòu)需仔細(xì)征詢(xún)既往病史和外出史,并加強(qiáng)宣教,將經(jīng)血傳播的疾病控制在源頭。
[Abstract]:Objective to determine the source of infection and transmission of falciparum malaria infected by blood transfusion. Methods the blood samples of the patients and 8 blood donors were tested for malaria, blood smear microscopic examination and Real-time PCR, and the positive blood samples were analyzed by molecular tracing and genotyping. Results both the patient and a homegrown blood donor from Africa showed falciparum malaria positive in rapid diagnostic test, blood smear microscopic examination and Real-time PCR test. The homology of the two SSU rRNA gene sequences was 100% PF EMP-1 genotyping showed that the blood samples of both recipients and donors were mixed infection of Plasmodium falciparum K1 and MD20 type. Confirmed that the patient's falciparum malaria was infected by transfusion of blood from returning blood donors in Africa. It is discussed that malaria screening is not included in the blood donation test in China, so it is easy to cause blood transfusion transmission malaria during the latent period or relapse of malaria donation. Blood collecting and supplying organizations should carefully consult the past medical history and travel history, and strengthen propaganda and education to control the blood-borne diseases at their source.
