慢性乙型肝炎患者肝細(xì)胞脂肪變性影響因素的分類樹模型分析

【摘要】:目的基于分類樹模型對慢性乙型肝炎(CHB)患者發(fā)生肝細(xì)胞脂肪變性的高危人群和影響因素進行研究,建立一種評估CHB患者發(fā)生肝細(xì)胞脂肪變性風(fēng)險的簡單方法。方法收集2006年1月-2014年12月在佛山市順德區(qū)第一人民醫(yī)院感染性疾病科住院,行肝穿刺活組織檢查的CHB患者的臨床資料和病理結(jié)果,利用分類樹模型分析肝細(xì)胞脂肪變性的影響因素,采用索引值曲線、錯分矩陣和估計誤差對分類樹模型分類效果進行整體評價。結(jié)果 CHB患者合并肝細(xì)胞脂肪變性的影響因素有BMI、總膽固醇、低密度脂蛋白,其中最重要的影響因素是BMI。該分類樹模型的敏感性為84.3%,特異性為81.5%,準(zhǔn)確率為82.9%,模型估計誤差為0.171,模型擬合效果較好。結(jié)論通過分類樹模型發(fā)現(xiàn)CHB患者發(fā)生肝細(xì)胞脂肪變性和BMI、總膽固醇、低密度脂蛋白等3個影響因素關(guān)系密切,根據(jù)這3個指標(biāo)可以建立一種簡單的分類方法來評估CHB患者發(fā)生肝細(xì)胞脂肪變性的風(fēng)險,有必要開展進一步臨床研究以明確其臨床應(yīng)用價值。
[Abstract]:Objective to study the high risk population and influencing factors of hepatocyte steatosis in patients with chronic hepatitis B (CHB) based on the classification tree model, and to establish a simple method for assessing the risk of hepatocyte steatosis in patients with chronic hepatitis B (CHB). Methods from January 2006 to December 2014, the clinical data and pathological results of CHB patients who were hospitalized in Department of Infectious Diseases, first people's Hospital of Shunde District, Foshan City, and underwent biopsy of liver biopsy were collected. The influencing factors of hepatocyte steatosis were analyzed by using the classification tree model. The classification effect of the classification tree model was evaluated by index curve, error matrix and estimation error. Results the influencing factors of CHB patients with hepatocyte fatty degeneration were BMI, total cholesterol and low density lipoprotein. The most important factor was BMI.. The sensitivity of the model is 84.3, the specificity is 81.5, the accuracy is 82.9, the model estimation error is 0.171, and the model fitting effect is better. Conclusion by using the classification tree model, it was found that there were close relationships among the three influencing factors of hepatocyte steatosis, BMI, total cholesterol and low density lipoprotein in patients with CHB. According to these three indexes, a simple classification method can be established to evaluate the risk of hepatocyte steatosis in patients with CHB. It is necessary to carry out further clinical research to determine its clinical application value.
