【摘要】：目的總結(jié)并分析結(jié)核性胸膜炎患者的B超影像特點(diǎn),研究B超在結(jié)核性胸膜炎診斷中的價(jià)值。方法選取2014年3月-2016年3月于醫(yī)院因疑似結(jié)核性胸膜炎接受B超和CT檢查的289例患者,將其中確診為滲出性結(jié)核性胸膜炎的共260例患者作為研究對(duì)象,分析滲出性結(jié)核性胸膜炎的B超圖像特點(diǎn),同時(shí)將病理診斷結(jié)果作為金標(biāo)準(zhǔn),對(duì)比B超檢查和CT檢查的特異性和靈敏度,分析B超檢查的診斷價(jià)值和效益成本。結(jié)果 B超檢查準(zhǔn)確率、特異度、靈敏度、陽性預(yù)測(cè)價(jià)值、陰性預(yù)測(cè)價(jià)值與CT檢查結(jié)果比較,差異無統(tǒng)計(jì)學(xué)意義;B超效益成本比顯著高于CT,兩組方法效益成本比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論不同類型的結(jié)核性胸膜炎患者,其B超圖像特點(diǎn)有一定差異,B超診斷結(jié)核性胸膜炎的準(zhǔn)確率、特異性、靈敏性均較為理想,且檢查價(jià)格低、重復(fù)性強(qiáng),具有較高臨床實(shí)際應(yīng)用價(jià)值。
[Abstract]:Objective to summarize and analyze the characteristics of B-ultrasound in tuberculous pleurisy and to study the value of B-ultrasound in the diagnosis of tuberculous pleurisy. Methods A total of 289 patients with suspected tuberculous pleurisy from March 2014 to March 2016 were examined by B-ultrasonography and CT. 260 of them were diagnosed as exudative tuberculous pleurisy. The characteristics of B-ultrasound images of exudative tuberculous pleurisy were analyzed. At the same time, the pathological diagnosis results were regarded as gold standard, the specificity and sensitivity of B-ultrasonography and CT were compared, and the diagnostic value and cost of B-ultrasonography were analyzed. Results the accuracy, specificity, sensitivity, positive predictive value and negative predictive value of B-ultrasound were not significantly different from those of CT, and the cost-benefit ratio of B-ultrasound was significantly higher than that of CT.There was a significant difference between the two groups. The difference was statistically significant (P 0.05). Conclusion different types of tuberculous pleurisy have different characteristics of B-ultrasound images. The accuracy, specificity and sensitivity of B-mode ultrasonography in the diagnosis of tuberculous pleurisy are ideal, and the price of examination is low and the reproducibility is strong. It has high clinical application value.
