【摘要】：目的比較瞬時(shí)彈性成像技術(shù)Fibro Touch與FIB-4對(duì)慢性乙型肝炎肝纖維化的診斷價(jià)值。方法收集肝臟病理符合慢性乙型肝炎的患者共計(jì)110例,Fibro Touch測量肝臟硬度值(LSM),同時(shí)完善FIB-4數(shù)值。結(jié)果 LSM數(shù)值與肝組織炎癥及纖維化分期呈正相關(guān)(r分別是0.532,0.670;均P0.001)。LSM數(shù)值在HBV DNA(copies/m L)10~3、10~3~10~5、10~5 3組間差異有統(tǒng)計(jì)學(xué)意義[(11.03±4.39)k Pa、(14.57±5.17)k Pa、(22.45±10.68)k Pa,P0.05]。FIB-4及Fibro Touch預(yù)測嚴(yán)重肝纖維化的AUROC分別為0.789(95%CI 0.711~0.854,P0.001)及0.866(95%CI 0.788~0.924,P0.001);預(yù)測早期肝硬化的AUROC分別為0.778(95%CI0.689~0.852,P0.001)及0.830(95%CI 0.746~0.895,P0.001)。結(jié)論 LSM數(shù)值與HBV DNA水平,肝臟炎癥及纖維化程度呈正相關(guān),Fibro Touch及FIB-4對(duì)于慢乙肝嚴(yán)重肝纖維化及肝硬化均具有良好的預(yù)測價(jià)值,Fibro Touch優(yōu)于FIB-4。
[Abstract]:Objective to compare the diagnostic value of transient elastic imaging (Fibro Touch) and FIB-4 in chronic hepatitis B liver fibrosis. Methods A total of 110 patients with chronic hepatitis B whose liver pathology was in accord with chronic hepatitis B were collected to measure the liver hardness and improve the FIB-4 value. Results there was a positive correlation between LSM value and hepatic inflammation and fibrosis stage (r = 0.532n0.670, respectively), and there was a significant difference in P0.001).LSM value between HBV DNA(copies/m group and HBV DNA(copies/m group (11.03 鹵4.39k Patius 14.57 鹵5.17k Pao 22.45 鹵10.68k P0. 05). FIB-4 and Fibro Touch in predicting severe hepatic fibrosis were 0.789(95%CI 0.7110.854mP0.001, respectively. The AUROC of early cirrhosis was 0.778T95 (CI 0.6890.852mP0.001) and 0.830(95%CI 0.7460.895g / P0.001g respectively. Conclusion the value of LSM is positively correlated with the level of HBV DNA, the degree of inflammation and fibrosis of liver. Fibro Touch and FIB-4 are good predictors of severe hepatic fibrosis and cirrhosis of chronic hepatitis B. Fibro Touch is superior to FIB-4.
