【摘要】：目的:通過分析37例口服魚膽中毒患者的臨床資料,研究魚膽中毒致臟器損傷臨床特征,探討有效的治療方法,指導(dǎo)臨床治療、評估預(yù)后。方法:收集2010年1月~2016年12月期間于吉林大學(xué)第一醫(yī)院住院的口服魚膽中毒患者的完整臨床資料,對患者的一般資料、臨床表現(xiàn)、化驗檢查指標(biāo)、治療方式、方法及轉(zhuǎn)歸進(jìn)行回顧性分析。結(jié)果:1、37例魚膽中毒患者經(jīng)治療后全部好轉(zhuǎn)出院,治愈率100%。2、進(jìn)食魚膽后癥狀最早發(fā)生于0.5小時,最晚發(fā)生在第10天。入院時主訴以“胃腸道癥狀”為主要表現(xiàn)者30例,以“無尿”或“少尿”為主訴者6例;以“發(fā)熱”為主要表現(xiàn)者1例。3、所有患者均有不同程度的肝功、腎功、心肌酶異常,且治療前后對比均有顯著性差異(P0.05)。4、器官損傷個數(shù)1~4個,2個臟器損傷者為數(shù)最多。部分患者出現(xiàn)漿膜腔積液(6例)、電解質(zhì)紊亂(14例)、尿路感染(4例)、高血壓(8例)、精神癥狀(10例),并有2例患者出現(xiàn)ARDS。5、30例給予血液凈化治療;7例拒絕血液凈化治療。HD、CRRT、CRRT后繼續(xù)規(guī)律HD以及CRRT直接聯(lián)合HP四種血液凈化方式相比較,其住院時間逐漸縮短。結(jié)論:1、魚膽中毒后臟器損傷數(shù)目與進(jìn)食魚膽數(shù)目不相關(guān);2、魚膽中毒后多以胃腸道癥狀為首發(fā)表現(xiàn),其后出現(xiàn)不同程度肝、腎、心、腦等器官損傷;3、魚膽中毒后及時就醫(yī)并經(jīng)積極保護(hù)臟器及對癥治療以及配合血液凈化治療后療效顯著;4、CRRT聯(lián)合HP血流動力學(xué)穩(wěn)定,并發(fā)癥少,有助于臟器功能恢復(fù),縮短住院時間;5、是否合并基礎(chǔ)疾病、年齡、器官損傷數(shù)目及血液凈化方式均可影響魚膽中毒患者預(yù)后,可作為評估魚膽中毒預(yù)后的參考指標(biāo)。
[Abstract]:Objective: to analyze the clinical data of 37 patients with fish bile poisoning, to study the clinical characteristics of visceral injury caused by fish bile poisoning, to explore the effective treatment method, to guide the clinical treatment and to evaluate the prognosis. Methods: from January 2010 to December 2016, we collected the complete clinical data of patients with oral fish gallbladder poisoning hospitalized in the first Hospital of Jilin University, and analyzed the general data, clinical manifestations, laboratory examination indexes and treatment methods of the patients. Methods and outcome were analyzed retrospectively. Results all 37 cases of fish bile poisoning were cured and discharged after treatment, the cure rate was 100. 2. The symptoms after eating fish bile occurred first at 0.5 hours, and the last occurred on the 10th day. At admission, the main complaints were "gastrointestinal symptoms" in 30 cases, "anuria" or "oliguria" in 6 cases, and "fever" in 1 case (.3). All the patients had different degrees of liver function, renal function and myocardial enzyme abnormality. There were significant differences before and after treatment (P 0.05. 4), the number of organ injury was 1 ~ 4, the number of two organ injuries was the most. There were 6 cases of serous cavity effusion, 14 cases of electrolyte disorder, 4 cases of urinary tract infection, 8 cases of hypertension, 10 cases of mental symptoms, and 2 cases of ARDS.530 cases. 7 cases refused blood purification treatment. Comparing the four ways of blood purification, HD and CRRT combined directly with HP after CRRT, The length of hospitalization was gradually shortened. Conclusion the number of organ injury after fish bile poisoning is not correlated with the number of fish bile intake. Gastrointestinal symptoms are the first manifestation of fish bile poisoning, and liver, kidney and heart appear in different degrees after fish bile poisoning. Brain and other organs were injured. After fish bile poisoning, the patients were treated with active organ protection, symptomatic treatment and blood purification therapy. The curative effect of CRRT combined with HP was stable, and the complications were less, which was helpful to the recovery of organ function. The prognosis of the patients with fish bile poisoning can be evaluated by shortening the hospitalization time or not, whether they are complicated with basic diseases, age, the number of organ damage and the blood purification mode, which can be used as a reference index to evaluate the prognosis of fish bile poisoning.
