【摘要】：目的 :探討肺血吸蟲病的臨床表現(xiàn)、影像學(xué)特征、診斷、鑒別診斷及治療。方法 :回顧性分析南京醫(yī)科大學(xué)第一附屬醫(yī)院呼吸科收治的1例肺血吸蟲病患者的臨床資料并進(jìn)行相關(guān)文獻(xiàn)復(fù)習(xí)。以"血吸蟲病"及"肺"為檢索詞檢索萬方數(shù)據(jù)庫和中國期刊網(wǎng)全文數(shù)據(jù)庫,檢索時間為2005年1月—2015年8月。結(jié)果:在以上數(shù)據(jù)庫共檢索到25篇相關(guān)文獻(xiàn)共177例肺血吸蟲病,包括本例在內(nèi)共178例,男132例,女46例,平均年齡32.6歲。該病誤診率較高,臨床主要表現(xiàn)為咳嗽、咳痰、發(fā)熱。影像學(xué)主要變現(xiàn)為肺紋理增多增粗、粟粒樣陰影、占位性病變。診斷主要依賴病理、血清學(xué)及體外孵化試驗(yàn)。多數(shù)患者經(jīng)驅(qū)蟲治療預(yù)后良好。結(jié)論:肺血吸蟲病臨床及影像學(xué)表現(xiàn)無特異性,需與肺結(jié)核、支氣管炎、肺部腫瘤等鑒別。對臨床表現(xiàn)為急、慢性感染性疾病經(jīng)抗感染治療效果欠佳者需警惕該病可能。
[Abstract]:Objective: to investigate the clinical manifestations, imaging features, diagnosis, differential diagnosis and treatment of pulmonary schistosomiasis. Methods: the clinical data of a case of lung schistosomiasis treated in the Department of Respiratory of the first affiliated Hospital of Nanjing Medical University were retrospectively analyzed and the related literatures were reviewed. Using "schistosomiasis" and "lung" as the key words to search Wanfang database and full text database of Chinese Journal Network, the retrieval time is from January 2005 to August 2015. Results: a total of 177 cases of lung schistosomiasis were found in 25 related literatures, including 178 cases (132 males and 46 females), with an average age of 32.6 years. The misdiagnosis rate of the disease is high. The main clinical manifestations are cough, expectoration and fever. The main imaging manifestations were increased and thickened lung texture, miliary shadow and space-occupying lesions. Diagnosis mainly depends on pathology, serology and in vitro incubation test. Most patients had a good prognosis after insect repellent treatment. Conclusion: the clinical and imaging findings of pulmonary schistosomiasis are not specific and should be distinguished from pulmonary tuberculosis, bronchitis and lung tumor. Patients with acute clinical manifestations and chronic infectious diseases with poor anti-infection treatment should be on guard against the possibility of the disease.
