【摘要】：目的了解云南省玉溪市農(nóng)村地區(qū)手足口病流行的相關(guān)影響因素,為有效控制手足口病在農(nóng)村地區(qū)的暴發(fā)和流行提供依據(jù)。方法研究采用1∶1病例對(duì)照研究方法,選取疫情報(bào)告系統(tǒng)中2014年5月1日至12月31日期間云南省玉溪市人民醫(yī)院臨床或?qū)嶒?yàn)室診斷為手足口病的全部鄉(xiāng)鎮(zhèn)患兒進(jìn)行編號(hào),采用系統(tǒng)抽樣的方法確定了其中來(lái)自農(nóng)村地區(qū)的205例病例,按照1∶1配對(duì)原則確定205例未患手足口的兒童為對(duì)照,采用問(wèn)卷調(diào)查的方法收集資料。用Epi Data 3.0建立數(shù)據(jù)庫(kù),SPSS 17.0統(tǒng)計(jì)軟件對(duì)數(shù)據(jù)進(jìn)行單因素、多因素條件Logistic回歸分析。結(jié)果 Logistic多因素回歸分析顯示,在控制了各變量間的混雜作用影響后,兒童與手足口病患兒有接觸史是流行的危險(xiǎn)因素(OR=6.622),而兒童飯前洗手和接受母乳喂養(yǎng)為保護(hù)因素(OR分別為0.485、0.396)。結(jié)論相比城鎮(zhèn)地區(qū),農(nóng)村地區(qū)兒童的手足口病防治應(yīng)該得到更多關(guān)注,培養(yǎng)成良好的衛(wèi)生習(xí)慣、提倡母乳喂養(yǎng)及避免讓兒童接觸手足口病患者是控制農(nóng)村手足口病的關(guān)鍵。
[Abstract]:Objective to investigate the related factors of HFMD prevalence in rural areas of Yuxi, Yunnan Province, and to provide evidence for the effective control of HFMD outbreaks and epidemics in rural areas. Methods the 1:1 case-control study was used to identify all the township children with HFMD diagnosed by clinic or laboratory in Yuxi people's Hospital, Yunnan Province from May 1 to December 31, 2014 in the epidemic situation reporting system. A systematic sampling method was used to determine 205 cases from rural areas and 205 children without hand and foot mouth as control according to 1:1 pairing principle. The data were collected by questionnaire. The statistical software SPSS17.0 of Epi Data 3.0 was used to analyze the data by univariate and multivariate conditional Logistic regression analysis. Results Logistic multivariate regression analysis showed that after controlling for the influence of the mixed effects of various variables, the history of contact between children and children with HFMD was an epidemic risk factor of OR6.622, while the OR of washing hands before meals and receiving breast feeding were 0.485 and 0.396respectively. Conclusion compared with urban areas, more attention should be paid to the prevention and treatment of HFMD in rural areas, and the key to control HFMD in rural areas is to cultivate good hygiene habits, to advocate breast-feeding and to avoid contact with HFMD patients in rural areas.
