【摘要】：目的采用薈萃分析的方法評價血清乳膠凝集試驗對肺隱球菌病的診斷價值,為臨床早期診斷提供參考依據(jù)。方法通過計算機檢索、中國知網(wǎng)、萬方、維普和中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫,篩選出有關(guān)血清乳膠凝集試驗診斷肺隱球菌病的研究,檢索日期從建庫開始截止至2015年8月,采用QUADAS量表對納入的文獻(xiàn)進(jìn)行質(zhì)量評價,使用薈萃Disc 1.4軟件檢驗異質(zhì)性并合并各檢驗效應(yīng)量,Stata 12.0軟件檢測發(fā)表偏倚。結(jié)果最終納入7篇文獻(xiàn);薈萃分析結(jié)果顯示,靈敏度為0.81,特異度為0.99,陽性似然比為63.8,陰性似然比為0.22,診斷優(yōu)勢比為343.74,SROC曲線下面積為0.997,Q值為0.982。結(jié)論血清乳膠凝集試驗診斷肺隱球菌病有較高的靈敏度及特異度,可作為早期快速診斷肺隱球菌病的重要方法。
[Abstract]:Objective to evaluate the diagnostic value of serum latex agglutination test for pulmonary Cryptococcus pneumoniae by meta-analysis. Weip and Chinese biomedical literature database were used to screen out the diagnosis of pulmonary cryptococcosis by serum latex agglutination test. The retrieval date was from the start of construction to August 2015. The quality of the included literature was evaluated by QUADAS scale. Meta Disc 1.4 software was used to test heterogeneity and combined with each test effect quantity. Finally, the results were included in 7 articles. The results of meta-analysis showed that, The sensitivity was 0.81, the specificity was 0.99, the positive likelihood ratio was 63.8, the negative likelihood ratio was 0.22, and the diagnostic odds ratio was 343.74. The area under SROC curve was 0.997Q was 0.982.Conclusion Serum latex agglutination test has higher sensitivity and specificity in the diagnosis of pulmonary cryptococcosis. It can be used as an important method for early rapid diagnosis of pulmonary cryptococcosis.
