【摘要】：【目的】 1.了解HIV-1慢性感染者外周血Th22、Th17、Th1、Treg細(xì)胞的變化情況及Th22、Th17、Th1與Treg之間平衡狀態(tài)，初步探討體內(nèi)輔助性T細(xì)胞功能亞群及其免疫平衡變化與HIV-1慢性感染者疾病進(jìn)展關(guān)系。 2.分析HIV-1慢性感染者短程抗病毒治療前后Th22、Th17、Th1、Treg細(xì)胞及其免疫平衡狀態(tài)的變化，初步探討抗病毒治療對(duì)免疫功能及免疫平衡狀態(tài)的影響。 【方法】 選取40例抗HIV-1陽性且CD4+T細(xì)胞350個(gè)/μl的感染者為研究組，予以初始抗病毒治療，采集治療前及治療1月、2月和3月外周血，同時(shí)選取30例健康人群為對(duì)照組，運(yùn)用細(xì)胞因子染色方法，流式細(xì)胞儀檢測Th22、Th17、Th1、Treg細(xì)胞表達(dá)所占百分比情況，分析比較研究組與對(duì)照組及研究組短程ART治療后各細(xì)胞表達(dá)的差異及相互間失衡情況。 【結(jié)果】 1. HIV-1慢性感染者抗病毒治療前后淋巴細(xì)胞計(jì)數(shù)及病毒載量變化情況 40例HIV-1慢性感染者抗病毒治療3月CD4+T細(xì)胞計(jì)數(shù)較治療前顯著增高(301.00±116.49個(gè)/μl vs.211.37±84.55個(gè)/μl，P=0.0002)，病毒載量log10水平顯著降低(5.48±0.76copies/ml vs.2.70copies/ml，P0.0001)。 2. HIV-1慢性感染者ART前后外周血Th22、Th17、Th1、Treg細(xì)胞百分比變化 HIV-1慢性感染者外周血Th22、Th17、Th1細(xì)胞百分比顯著低于健康對(duì)照（0.59±0.47%vs.1.65±0.56%，P0.0001；4.46±1.84%vs.6.98±1.86%，P0.0001；16.75±6.72%vs.22.77±6.87%，P=0.0005）。Th22、Th17細(xì)胞百分比隨抗病毒治療時(shí)間的延長逐漸恢復(fù)，Th22細(xì)胞百分比在治療2月、Th17細(xì)胞百分比在治療3月時(shí)接近健康對(duì)照（1.25±1.1%vs.1.65±0.56%，P=0.0725；6.33±2.64%vs.6.98±1.86%，P=0.2571），Th1細(xì)胞百分比在治療前后無顯著變化，均低于健康對(duì)照。而Treg細(xì)胞百分比顯著高于健康對(duì)照（10.76±3.76%vs.7.01±1.88%，P0.0001），并隨抗病毒治療時(shí)間的延長逐漸降低，治療2月時(shí)較治療前顯著降低（9.22±2.56%vs.10.76±3.76%，P=0.035），但仍高于健康對(duì)照組。 3. HIV-1慢性感染者外周血Th22、Th17、Th1、Treg細(xì)胞百分比與CD4+T細(xì)胞計(jì)數(shù)、病毒載量相關(guān)性 HIV-1慢性感染者外周血Treg細(xì)胞百分比與CD4+T細(xì)胞計(jì)數(shù)顯著負(fù)相關(guān)（r=-0.6574，P0.0001），與病毒載量顯著正相關(guān)（r=0.4572，P=0.003）。Th22、Th17、Th1細(xì)胞百分比與CD4+T細(xì)胞計(jì)數(shù)及病毒載量無明顯相關(guān)。 4. HIV-1慢性感染者ART前后外周血Th22、Th17、Th1與Treg細(xì)胞免疫失衡情況 HIV-1慢性感染者外周血Th22/Treg、Th17/Treg、Th1/Treg比值均顯著低于健康對(duì)照（0.05±0.03vs.0.25±0.1，P0.0001；0.46±0.27vs.1.07±0.42，P0.0001；1.56±0.89vs.3.37±1.02，P0.0001），隨抗病毒治療時(shí)間延長顯著恢復(fù)。治療3月較治療前顯著恢復(fù)（0.17±0.1vs.0.05±0.03，P0.0001；0.81±0.46vs.0.46±0.27，P0.0001；2.31±1.27vs.1.56±0.89，P=0.0033），但仍顯著低于健康對(duì)照。 5. HIV-1慢性感染者外周血Th22、Th17、Th1與Treg細(xì)胞免疫失衡與CD4+T細(xì)胞計(jì)數(shù)、病毒載量相關(guān)性分析 HIV-1慢性感染者外周血Th17/Treg比值與CD4+T細(xì)胞計(jì)數(shù)顯著正相關(guān)（r=0.3125，P=0.0496），與病毒載量顯著負(fù)相關(guān)（r=-0.4747，P=0.002）。Th1/Treg比值與CD4+T細(xì)胞計(jì)數(shù)顯著正相關(guān)（r=0.3335，P=0.0355），，與病毒載量無明顯相關(guān)。Th22/Treg比值與CD4+T細(xì)胞計(jì)數(shù)及病毒載量均無顯著相關(guān)。 【結(jié)論】 1．HIV-1慢性感染者外周血Th22、Th17、Th1細(xì)胞百分比明顯下降，有效抗病毒治療有助于免疫功能重建。 2．HIV-1慢性感染者外周血Treg細(xì)胞百分比升高，經(jīng)有效抗病毒治療可降低，抗病毒治療可調(diào)節(jié)免疫狀態(tài)。 3. HIV-1慢性感染者外周血Th22、Th17、Th1與Treg細(xì)胞平衡被破壞，且與CD4+T細(xì)胞及病毒載量相關(guān)，預(yù)示輔助性T細(xì)胞亞群間平衡狀態(tài)與疾病進(jìn)展密切相關(guān)。 4. HIV-1慢性感染者經(jīng)有效抗病毒治療后外周血Th22、Th17、Th1與Treg細(xì)胞間失衡狀態(tài)逐漸恢復(fù)，表明抗病毒治療有助于恢復(fù)體內(nèi)免疫平衡。
[Abstract]:Purpose of the project

1 . To investigate the changes of Th22 , Th17 , Th1 , Treg cells in peripheral blood of HIV - 1 patients with chronic infection and Th22 , Th17 , Th1 and Treg .

2 . The changes of Th22 , Th17 , Th1 , Treg cells and their immune equilibrium state before and after short - range antiviral therapy of HIV - 1 chronic infection were analyzed , and the effects of antiviral therapy on the immune function and immune equilibrium state were discussed .

Methodology

Forty patients with HIV - 1 positive and 350 / 渭l of CD4 + T cells were selected as the study group , and the initial antiviral therapy was given , and the peripheral blood was collected before and after treatment in January , February and March .
