【摘要】：目的探討烏魯木齊市男男性行為者(MSM)HIV感染狀況及其影響因素,為針對(duì)性開(kāi)展MSM高危性行為干預(yù)提供依據(jù)。方法 2013年5—8月在烏魯木齊市天山區(qū)和沙依巴克區(qū)通過(guò)現(xiàn)場(chǎng)、網(wǎng)絡(luò)和社會(huì)小組方式招募年齡18周歲、自述有同性性行為、自愿接受調(diào)查的MSM進(jìn)行問(wèn)卷調(diào)查、采血和檢測(cè)。結(jié)果共調(diào)查857例MSM,維吾爾族HIV陽(yáng)性檢出率為13.1%(14/107),高于漢族的7.2%(32/445)(χ2=3.922,P=0.048)。近6個(gè)月內(nèi)發(fā)生同性性行為者804例(93.8%)、僅與固定同性發(fā)生性行為者173例(20.2%)、與同性和異性發(fā)生性行為者107例(12.5%),發(fā)生以上3種性行為時(shí)安全套使用率分別為38.3%(308/804)、77.5%(134/173)、28.0%(30/107);近6個(gè)月內(nèi)購(gòu)買(mǎi)、提供同性商業(yè)性行為者分別占3.9%(33/857)、1.8%(15/857),性交時(shí)安全套使用率分別為84.8%(28/33)、66.7%(10/15);HIV抗體檢出率為8.3%(71/857)。多因素Logistic回歸分析顯示,近6個(gè)月內(nèi)發(fā)生同性性行為〔OR=5.820,95%CI(1.302,26.015)〕、近6個(gè)月內(nèi)與非固定同性性伴發(fā)生無(wú)保護(hù)性性行為〔OR=2.475,95%CI(1.255,4.879)〕、梅毒感染陽(yáng)性〔OR=4.208,95%CI(1.979,8.947)〕是烏魯木齊市MSM HIV感染的影響因素。結(jié)論烏魯木齊市MSM高危性行為普遍,不同民族MSM中HIV感染率、性行為方式有差異,人群活動(dòng)隱蔽、高危性行為頻繁和艾滋病預(yù)防服務(wù)缺乏是MSM HIV感染的主要原因。
[Abstract]:Objective to investigate the HIV infection status and its influencing factors among men who have sex with men in Urumqi, and to provide the basis for the intervention of MSM high-risk sexual behaviors. Methods from May to August, 2013, the HIV infection in Tianshan District and Shayibak District of Urumqi City was passed through the scene. Online and social group recruitment of 18 years of age, self-reported same-sex behavior, voluntary survey of the MSM questionnaire, Results A total of 857 cases of MSM were investigated. The positive rate of HIV in Uygur was 14 / 107, which was higher than that of the Han nationality (7.22 / 445) (蠂 ~ 2 ~ 2 ~ 3.922P ~ (0.048)). In the last 6 months, 804 cases of same-sex sex occurred in 93.8%, only with fixed same-sex sex (173 cases) with 20.2T, and with the same sex and heterosexual sex. The condom use rate of 107 cases of sexual behavior was 38.3 / 804 / 77.50.13.4 / 173/ 28.0 / 107respectively, and purchased in the last six months, and the rate of condom use was 38.3 / 804 / 77.5and 28.0 / 107respectively when the above three kinds of sexual behaviors occurred; in the past six months, the rate of condom use was 38.3% and 28.0% / 107%, respectively. The proportion of those who provided commercial sex with the same sex was 3.9%. The proportion of those who provided commercial sex with the same sex was 3.3% 857% or 1.8%, and the rate of condom use at sexual intercourse was 84.8% and 66.7%, respectively. The detection rate of anti-HIV antibody was 8.3% / 857%. Multivariate Logistic regression analysis showed that. In the last 6 months, the incidence of same-sex sex was 5.820 ~ 95CII 1.30226.0155. in the last 6 months, the incidence of unprotected sex with non-fixed same-sex partners was 1.25554.879C, and the positive rate of syphilis infection was OR4.20895CI1.9798.9477. conclusion the high risk behaviors of MSM in Urumqi are common. The main causes of MSM HIV infection were the infection rate of HIV in different nationalities, the difference of sexual behavior, the covert activity of people, the frequent high risk sexual behavior and the lack of AIDS prevention services.
