福建省EV71流行與遺傳變異特點(diǎn)分析

【摘要】:目的分析2010-2015年福建省腸道病毒71型(EV71)流行概況及病毒VP1基因特征,為今后EV71疫苗在福建省的應(yīng)用提供參考。方法基于手足口病監(jiān)測(cè)系統(tǒng),對(duì)福建省2010-2015年間EV71流行情況進(jìn)行描述性流行病學(xué)分析。隨機(jī)選取72株病毒分離株,擴(kuò)增病毒VP1區(qū)并與疫苗株作序列進(jìn)化分析。比較兩者核苷酸及氨基酸序列差異性,分析分離株在VP1蛋白4個(gè)免疫表位的氨基酸變異情況。結(jié)果 2010-2015年,EV71總體感染率呈下降趨勢(shì),在每年的5-7月均出現(xiàn)一次流行高峰;發(fā)病地區(qū)主要集中在廈門、福州、南平、泉州;主要感染1-3歲年齡段兒童,男性比例高于女性,職業(yè)分布以散居兒童為主。重癥病例中EV71比例高于在總病例中的比例(χ2=732.064 5,P0.000 1)。福建地區(qū)流行的EV71病毒均屬于C4a亞型,與疫苗株的核苷酸差異性隨時(shí)間距離增加而逐漸增大,氨基酸差異性未有明顯變化;在4個(gè)免疫表位上的氨基酸變異未發(fā)現(xiàn)年度上的規(guī)律和同HMFD中EV71構(gòu)成分布相符的變化。結(jié)論 2010年至2015年,福建地區(qū)流行的EV71具有明顯的時(shí)間、地點(diǎn)、人群分布特征,72株病毒株均屬于C4a基因亞型,與疫苗株相比,VP1區(qū)未出現(xiàn)顯著的抗原漂移及變異。應(yīng)繼續(xù)保持對(duì)EV71流行病學(xué)監(jiān)測(cè)和遺傳特征的分析,為今后疫苗的應(yīng)用提供更為詳盡的資料。
[Abstract]:Objective to analyze the prevalence of enterovirus 71 (EV71) and the characteristics of VP1 gene in Fujian Province from 2010 to 2015. To provide a reference for the future application of EV71 vaccine in Fujian Province. Methods based on the hand-foot-mouth disease surveillance system. The epidemic situation of EV71 in Fujian Province from 2010 to 2015 was analyzed by descriptive epidemiology. 72 strains of virus were randomly selected. The VP1 region of the virus was amplified and the nucleotide and amino acid sequence difference was compared with that of the vaccine strain. The amino acid variation of the four immune epitopes of VP1 protein was analyzed. Results the overall infection rate of EV71 decreased from 2010 to 2015. An epidemic peak appeared in May-July of each year. The incidence areas were mainly in Xiamen, Fuzhou, Nanping, Quanzhou; The proportion of males was higher than that of females, and the occupational distribution was mainly scattered children. The proportion of EV71 in severe cases was higher than that in total cases (蠂 2 732 064 5). The nucleotide difference between P0. 000 1 and vaccine strain increased with time, but the difference of amino acid did not change significantly in Fujian province. All of the EV71 viruses in Fujian were C4a subtype, and the nucleotide difference increased gradually with the increase of time distance between the EV71 virus and the vaccine strain. The amino acid variation in the four immune epitopes was not found to be consistent with the distribution of EV71 composition in HMFD. Conclusion from 2010 to 2015. The prevalence of EV71 in Fujian has obvious time, location and population distribution characteristics. 72 strains belong to C4a gene subtype, compared with vaccine strain. There was no significant antigen drift and variation in the VP1 region. The epidemiological monitoring and genetic analysis of EV71 should be maintained so as to provide more detailed information for the application of vaccines in the future.
