PCR-線性探針雜交酶顯色法在結核病診斷與耐藥性檢測中的應用研究

【摘要】:目的對比PCR-線性探針雜交酶顯色法(簡稱線性探針法)與BACTEC MGIT960液體培養(yǎng)及藥敏在結核分枝桿菌檢出率的差異,評價兩種方法檢測結核分枝桿菌對利福平和異煙肼耐藥性的應用效果。方法選取醫(yī)院結核科2013年8月-2015年5月369例確診及疑似結核病的住院患者送檢標本,同時采用線性探針法和BACTEC MGIT960對其進行結核分枝桿菌及利福平和異煙肼耐藥性檢測,數據采用SPSS 13.0軟件進行統(tǒng)計分析。結果線性探針法檢測結核分枝桿菌對利福平耐藥性的靈敏度為90.4%、特異度為97.3%、Kappa值為0.872;對異煙肼耐藥性檢測的靈敏度為86.6%、特異度為96.3%、Kappa值為0.841,兩種方法在利福平和異煙肼耐藥性檢測中具有較好的一致性。結論 PCR-線性探針雜交酶顯色法是一種快速、準確的結核分枝桿菌及其對利福平和異煙肼耐藥性的診斷方法,對耐多藥結核病的快速診斷具有顯著的優(yōu)點。
[Abstract]:Objective to compare the PCR- linear probe hybridization enzyme colorimetric method (the linear probe) the difference in detection rate and BACTEC MGIT960 liquid culture and drug resistance in Mycobacterium tuberculosis, to evaluate the application effect of two methods for detection of Mycobacterium tuberculosis resistance to rifampin and isoniazid. Methods selected the hospital department of tuberculosis in August 2013 -2015 year in May 369 cases and the samples were hospitalized with suspected tuberculosis patients, at the same time using a linear probe and BACTEC MGIT960 for detection of Mycobacterium tuberculosis resistance to rifampin and isoniazid and the SPSS 13 software was used for statistical analysis. The results of detection of Mycobacterium tuberculosis rifampin resistance probe method of linear sensitivity was 90.4%, specificity was 97.3%, the Kappa value is 0.872 the sensitivity to isoniazid resistance; detection was 86.6%, specificity was 96.3%, the Kappa value is 0.841, two kinds of methods in rifampin and isoniazid Conclusion: PCR- linear probe hybridization enzyme chromogenic assay is a rapid and accurate method for the diagnosis of Mycobacterium tuberculosis and its resistance to rifampicin and isoniazid, and has a significant advantage for the rapid diagnosis of multidrug-resistant tuberculosis.
