膽堿能系統(tǒng)參與SCS鎮(zhèn)痛作用機(jī)制的實(shí)驗(yàn)研究

【摘要】:本研究通過(guò)建立與人類(lèi)神經(jīng)源性疼痛高度相近的坐骨神經(jīng)損傷致疼痛模型,根據(jù)坐骨神經(jīng)損傷后大鼠疼痛行為學(xué)的變化以及對(duì)脊髓刺激(SCS)反應(yīng)的有效性或無(wú)效性進(jìn)行分組,應(yīng)用微量分析方法、高壓液相色譜分析法和拮抗劑阻斷實(shí)驗(yàn)觀察SCS過(guò)程中脊髓后角內(nèi)乙酰膽堿濃度的變化以及SCS激活的乙酰膽堿受體亞型情況,以初步判定脊髓膽堿能神經(jīng)系統(tǒng)在SCS中的作用;同時(shí)利用免疫組織化學(xué)方法檢測(cè)不同組實(shí)驗(yàn)動(dòng)物脊髓內(nèi)膽堿能受體亞型及相關(guān)遞質(zhì)5-HT表達(dá)的變化,進(jìn)一步探討其在SCS中的作用。結(jié)果表明,脊髓內(nèi)膽堿能神經(jīng)系統(tǒng)與SCS鎮(zhèn)痛機(jī)制密切相關(guān),尤其是M4型毒蕈堿受體;脊髓后角內(nèi)Ach的水平與脊髓刺激有效性成正相關(guān)。本研究為臨床應(yīng)用SCS治療神經(jīng)源性疼痛提供了更深度的理論依據(jù)和治療靶點(diǎn)。
[Abstract]:In this study, the pain model of sciatic nerve injury was established, which was similar to that of human neuropathic pain, and was divided into groups according to the changes of pain behavior and the response of (SCS) to spinal cord stimulation in rats after sciatic nerve injury. The changes of acetylcholine concentration in the posterior horn of spinal cord and the acetylcholine receptor subtypes activated by SCS during SCS were observed by microanalysis, high pressure liquid chromatography and antagonist blocking experiments. The role of spinal cholinergic nervous system in SCS was preliminarily determined. The expression of cholinergic receptor subtypes and related transmitters 5-HT in spinal cord of different groups of experimental animals were detected by immunohistochemical method, and the role of cholinergic receptor subtypes in SCS was further discussed. The results showed that the cholinergic nervous system in the spinal cord was closely related to the analgesic mechanism of SCS, especially the type M4 muscarinic receptor, and the level of Ach in the posterior horn of the spinal cord was positively correlated with the effectiveness of spinal cord stimulation. This study provides a deeper theoretical basis and therapeutic target for the clinical application of SCS in the treatment of neuropathic pain.
