gyrA、gyrB和外排系統(tǒng)共同介導(dǎo)銅綠假單胞菌對喹諾酮類耐藥機制研究

【摘要】:目的探討臨床分離的對環(huán)丙沙星和左氧氟沙星均耐藥的銅綠假單胞菌耐藥機制。方法收集臨床分離經(jīng)VITEK-2(C0mpact細菌鑒定儀檢測環(huán)丙沙星和左氧氟沙星均耐藥的銅綠假單胞菌,瓊脂稀釋法測定環(huán)丙沙星和左氧氟沙星的MIC值,PCR擴增DNA促旋酶基因gyrA和gyrB以及DNA拓撲異構(gòu)酶Ⅳ的parC和parE基因,實時-RT-PCR分析細菌外排系統(tǒng)表達情況。結(jié)果瓊脂稀釋法檢測結(jié)果與VITEK-2 Compact細菌鑒定儀檢測結(jié)果相符。PCR擴增測序發(fā)現(xiàn)以DNA促旋酶基因gyrA(在位點941處插入堿基C)和gyrB(3株在位點1588處缺失堿基A,其他菌株在位點1543處插入堿基T)基因缺失或者插入導(dǎo)致移碼突變?yōu)橹?parE基因有3株在位點1895處插入堿基C。實時定量PCR檢測發(fā)現(xiàn)以mexA和mexC表達增加為主。結(jié)論檢出的耐環(huán)丙沙星和左氧氟沙星銅綠假單胞菌是由于DNA促旋酶基因gyrA和gyrB基因的突變和mexAB-OprM和mexCD-OprJ表達增加共同作用的結(jié)果。
