腰椎峽部裂修復(fù)重建手術(shù)的應(yīng)用解剖學(xué)研究

【摘要】:研究設(shè)計: 對成年人L4,5椎骨橫突、椎板、峽部及與其前方脊神經(jīng)前支的關(guān)系進(jìn)行大體解剖學(xué)測量,并對L4,5椎骨橫突內(nèi)部結(jié)構(gòu)進(jìn)行硬組織切片觀察。 研究目的: 為腰椎峽部裂的修復(fù)重建手術(shù)提供解剖學(xué)依據(jù),以提高手術(shù)的安全性和有效性。 研究背景: 峽部裂的修復(fù)重建手術(shù)由于能夠最大限度保留脊柱的運(yùn)動功能,更加符合人體的生理狀態(tài),故近年來迅速發(fā)展。但國內(nèi)由于缺乏國人的相關(guān)應(yīng)用解剖學(xué)數(shù)據(jù)的指導(dǎo)和支持,臨床醫(yī)師在實施該手術(shù)時只能憑借經(jīng)驗進(jìn)行操作,無法確保操作的精確性和安全性,以及所選用器材尺寸的合理性。 材料與方法: 對31具成年人L4,5椎骨骼標(biāo)本進(jìn)行解剖測量,測量指標(biāo)如下:(1)橫突:雙側(cè)橫突長度,橫突起始部寬度,橫突內(nèi)1/4點處寬度,橫突中點處寬度,橫突起始部厚度,橫突內(nèi)1/4點處厚度,橫突中點處厚度。(2)椎板:椎板中部高度,椎板上緣中點處厚度,椎板下緣中點處厚度,椎板中部厚度,椎板與棘突根部交線中點到對側(cè)上關(guān)節(jié)突起始部前緣的距離,椎板與棘突根部交線中點到對側(cè)橫突起始部前緣的距離,椎板與棘突根部交線中點到對側(cè)下關(guān)節(jié)突起始部前緣的距離,正上方觀釘?shù)琅c矢狀面的夾角,正后方觀釘?shù)琅c矢狀面的夾角。(3)峽部:峽部中點處的高度,峽部上緣與上關(guān)節(jié)突交界處的厚度,峽部下緣與下關(guān)節(jié)突交界處的
[Abstract]:Research and design:. The transverse process, lamina, isthmus and the anterior branch of the anterior spinal nerve were measured and the internal structure of the transverse process was observed by hard tissue section. Objectives of the study:. To provide anatomical basis for the repair and reconstruction of lumbar spondylolysis, in order to improve the safety and effectiveness of the operation. Background:. The repair and reconstruction operation of isthmus bifida has been developed rapidly in recent years because it can preserve the motor function of the spine to the maximum extent and is more in line with the physiological state of the human body. However, there is a lack of guidance and support from the relevant applied anatomical data in China. The clinician can only use experience to perform the operation, which can not ensure the accuracy and safety of the operation, and the rationality of the size of the equipment selected. Materials and methods:. The anatomical measurements were performed on 31 adult L4 / 5 vertebrae specimens. The measurement indexes were as follows: bilateral transverse process length, width of the beginning of transverse process, width of 1 / 4 point of transverse process, width of midpoint of transverse process, thickness of beginning part of transverse process. The height of the middle of the lamina, the thickness of the midpoint of the upper margin of the lamina, the thickness of the middle point of the lower margin of the lamina, the thickness of the middle point of the lamina, the thickness of the middle part of the lamina, the thickness of the middle point of the transverse process, and the thickness of the midpoint of the transverse process. The distance from the midpoint of the intersecting line between the lamina and the spinous process to the front of the origin of the contralateral superior articular process, and the distance between the midpoint of the intersecting line between the lamina and the root of the spinous process and the front edge of the opposite lateral transverse process. The distance from the midpoint of the intersecting line between the lamina and the spinous process to the anterior edge of the starting part of the contralateral inferior articular process, the angle between the upper epigastric snail and the sagittal plane, and the angle between the posterior posterior snail and the sagittal plane. The height of the isthmus at the midpoint of the isthmus. The thickness at the junction of the upper edge of the isthmus with the superior articular process, and the junction of the lower edge of the isthmus with the inferior articular process.
