國人脛骨近端形態(tài)學(xué)測量與研究

【摘要】: 目的測量中國人脛骨近端形態(tài)學(xué)指標(biāo)的數(shù)據(jù),為手術(shù)確定具有臨床意義的觀測指標(biāo),進(jìn)一步研制符合中國人脛骨近端解剖形態(tài)的內(nèi)固定物提供依據(jù)。方法上海或華東地區(qū)的成年健康人及骨關(guān)節(jié)炎患者共112人(158膝),年齡為20-88歲,平均年齡56歲。其中健康人80膝,骨關(guān)節(jié)炎患者78膝。受試者拍攝下肢站立位全長X線片,并按要求測量髖-膝-踝角(HKA)、脛股角(FTA)和脛骨平臺內(nèi)翻角(TP-TSA),再分別用脛骨中上段前側(cè)骨皮質(zhì)切線、脛骨中上段軸線和脛骨中上段后側(cè)骨皮質(zhì)切線為參考線分別測量脛骨平臺后傾角(PS)。結(jié)果健康人HKA平均177.4°±1.5°,FTA平均175.5°±2.4°,TP-TSA平均84.6°±2.5°,三種方法測量內(nèi)側(cè)PS分別為14.0°±3.0°、11.0°±2.7°、8.0°±2.9°,外側(cè)PS分別為11.2°±3.1°、8.1°±2.9°、5.3°±2.9°。經(jīng)統(tǒng)計學(xué)分析證實健康人與骨關(guān)節(jié)炎患者的HKA、FTA之間差異有統(tǒng)計學(xué)意義,而對于骨關(guān)節(jié)炎程度較重的患者,其PS會顯著減小。三種測量方法及內(nèi)外側(cè)平臺之間測得的數(shù)據(jù)差異均有統(tǒng)計學(xué)意義。結(jié)論中國人脛骨平臺的內(nèi)翻及后傾傾向比西方人更為顯著,FTA、TP-TSA和PS在手術(shù)和臨床研究都具有很大的實用價值。
[Abstract]:Objective to measure Chinese proximal tibial morphological index data, determine the observation index has clinical significance for the surgery, with the further development of Chinese internal fixation of proximal tibial anatomical basis. Adult healthy people and patients with osteoarthritis in Shanghai or the East China area a total of 112 people (158 knees), aged 20-88 the average age of 56 years. Among them, 80 healthy people of knee osteoarthritis patients, 78 knees. Subjects photographed standing full-length radiographs of lower extremity, and according to the requirements of measurement of hip knee ankle angle (HKA), femorotibial angle (FTA) and tibial plateau angle (TP-TSA), and then were used in tibia the upper anterior cortical bone tangent, upper tibia upper and posterior tibial axis in cortical tangent reference line were measured after tibial plateau angle (PS). The results of healthy human HKA with an average of 177.4 degrees - 1.5 degrees, the average FTA of 175.5 degrees plus or minus 2.4 degrees, TP-TSA averaged 84.6 degrees - 2.5 degrees, three kinds of measurement methods The medial PS were 14 degrees plus or minus 3 degrees, 11 degrees - 2.7 degrees, 8 degrees - 2.9 degrees lateral PS were 11.2 degrees plus or minus 3.1 degrees, 8.1 degrees - 2.9 degrees, 5.3 degrees - 2.9 degrees. That healthy people and patients with osteoarthritis HKA by statistical analysis, statistically significant differences between FTA the degree of osteoarthritis, heavier patients, the PS will be significantly reduced. Between the three kinds of measurement method and platform inside and outside of the measured data were statistically significant. Conclusion Chinese tibial plateau varus and posterior tilt tend to be more significant FTA than Westerners, TP-TSA and PS have great practical value in the surgical and clinical research.
