東北地區(qū)成人腰椎棘突傾斜角度數(shù)字解剖學:為椎弓根螺釘置入的參考標準

【摘要】:背景:經(jīng)后路椎弓根螺釘釘棒系統(tǒng)內(nèi)固定是各種腰椎手術的主要術式,其成功的關鍵是椎弓根螺釘?shù)臏蚀_置入,但棘突是位置表淺的解剖標志,在術中有較大的參考價值。目的:分析東北地區(qū)成人腰椎棘突傾斜情況,并進行解剖分級,同時討論其作為椎弓根螺釘內(nèi)固定手術局部解剖學定位標志參考的可行性。方法:對97例東北地區(qū)成人腰椎棘突傾斜角度進行測量,并根據(jù)傾斜角度進行分型;對各研究對象進行模擬手術,確定椎弓根螺釘?shù)淖罴阎冕斀嵌?測量椎弓根螺釘與棘突、標準矢狀面夾角,對二者進行差異性比較。結(jié)果與結(jié)論:(1)腰椎各型分布如下:型占75.1%,Ⅱ型占20.9%,Ⅲ型占2.3%,Ⅳ型占1.7%,經(jīng)比較椎弓根螺釘與棘突、標準矢狀面夾角無明顯差異;(2)結(jié)果說明,腰椎棘突傾斜角度存在不同程度變異,腰椎手術前應通過相關檢查明確變異情況,棘突可做為椎弓根螺釘置釘時傾斜角度的參考。
[Abstract]:Background: posterior pedicle screw system internal fixation is the main method of lumbar spine surgery. The key to its success is the accurate placement of pedicle screw, but the spinous process is the anatomic sign of superficial position and has great reference value during operation. Objective: to analyze the obliquity of spinous process of adult lumbar vertebrae in Northeast China, and to discuss the feasibility of using it as a reference for local anatomical localization of pedicle screw fixation. Methods: the inclination angle of lumbar spine process of 97 adults in Northeast China was measured and classified according to the angle of inclination, the optimal angle of pedicle screw was determined by simulated operation, and the pedicle screw and spinous process were measured.
