【摘要】：本文以敘事分析為切入點(diǎn)，以話語分析理論和疾病敘事為理論框架，從敘事學(xué)的角度分析醫(yī)患交際話語（尤其指中醫(yī)醫(yī)患交際會話）。傳統(tǒng)的醫(yī)患話語分析方法（即從語言學(xué)的角度研究）最近幾年在我國已經(jīng)有了不同程度的發(fā)展，國外也有些醫(yī)療從業(yè)人員從醫(yī)學(xué)角度分析醫(yī)患話語。因此筆者認(rèn)為話語分析的研究視角有待于進(jìn)一步的拓展與創(chuàng)新。而從敘事學(xué)的角度分析醫(yī)患話語為話語分析領(lǐng)域提供了一個新的研究視角和方法。 在臨床診斷的過程中，敘事提供了一個理解個人經(jīng)歷的方法，并且?guī)椭⒘酸t(yī)患之間的溝通和理解，同時提供了一個和疾病相關(guān)的有效的分析線索。它能夠系統(tǒng)地回答對于搜集信息來說什么是重要的，也能夠在合適的背景下接近受試者真實的心理過程，這樣能增強(qiáng)醫(yī)患之間對于疾病及其治療的理解，大大地豐富了話語分析理論在醫(yī)療實踐中的應(yīng)用。因此，結(jié)合敘事維度對醫(yī)患話語分析做一次整合研究，無論從話語分析的語言學(xué)發(fā)展趨勢還是從人文發(fā)展趨勢來看，都是非常必要的。 本研究正是借助敘事理論的指導(dǎo)，，探討分析的是醫(yī)患交際會話，尤其中醫(yī)醫(yī)患交際會話，因為國外的研究主要集中在西方醫(yī)學(xué)臨床實踐中，所以本研究就把重點(diǎn)放在中醫(yī)醫(yī)患交際的會話中。本研究結(jié)合了定量研究和微觀研究的方法，把搜集來的中醫(yī)臨床問診語料進(jìn)行轉(zhuǎn)寫和整理，在此基礎(chǔ)上嘗試提出兩種醫(yī)患交際的會話分析模式：（1）內(nèi)行患者型醫(yī)患會話模式；（2）共同構(gòu)建型醫(yī)患會話模式。然后利用這兩種模式對于不同類型患者的疾病敘事分別進(jìn)行分析，同時考慮并整合了其他諸如社會文化背景及敘事者個人心理特點(diǎn)等復(fù)雜因素，為話語分析提供了一個新的范例和視角。
[Abstract]:This paper takes narrative analysis as the breakthrough point, discourse analysis theory and disease narrative as the theoretical framework. From the perspective of narratology, this paper analyzes the doctor-patient communication discourse (especially the doctor-patient communication conversation in TCM). In recent years, the traditional methods of doctor-patient discourse analysis (that is, from the perspective of linguistics) have developed in different degrees in China. Some medical practitioners abroad analyze doctor-patient discourse from a medical point of view. Therefore, the author thinks that the perspective of discourse analysis needs to be further expanded and innovated.However, from the perspective of narratology, the analysis of doctor-patient discourse is discourse analysis. The field provides a new perspective and method for research. In the course of clinical diagnosis, narration provides a way to understand personal experiences, and helps to establish communication and understanding between doctors and patients. It also provides an effective analytical clue related to disease. It can systematically answer what is important for gathering information, and can approach the real psychological process of the subjects in the right context. This can enhance the understanding of disease and its treatment between doctors and patients, and enrich the application of discourse analysis theory in medical practice. It is necessary both from the linguistic trend of discourse analysis and from the trend of humanistic development. Under the guidance of narrative theory, this study explores and analyzes the doctor-patient communication conversation, especially the doctor-patient communication conversation in traditional Chinese medicine, because the research abroad is mainly focused on the clinical practice of western medicine. Therefore, this study focuses on the conversation of doctor-patient communication in TCM. This study combines quantitative and microscopic research methods to transfer and collate the collected Chinese medicine clinical inquiry materials. On the basis of this, we try to put forward two kinds of conversational analysis mode of doctor-patient communication, namely, "1")) to construct the "doctor-patient conversation" model together. Then, the two models are used to analyze the disease narrative of different types of patients, respectively. At the same time, other complex factors such as sociocultural background and narrator's psychological characteristics are considered and integrated, which provides a new paradigm and perspective for discourse analysis.
