質(zhì)譜分析在發(fā)現(xiàn)胃早癌標(biāo)志物的應(yīng)用

背景胃癌在目前所有惡性腫瘤中發(fā)病率位列第五,死亡率位列第三。我國的胃癌發(fā)病率和死亡率均處于世界較高水平,每年新發(fā)病例約40萬例,占世界總發(fā)病例數(shù)的42%。早期發(fā)現(xiàn)、早期診斷和早期治療是有效改善胃癌預(yù)后的方法。消化內(nèi)鏡的診斷效果較好,但作為有創(chuàng)操作,繁瑣耗時,不適合大規(guī)模人群篩查。近年來,對胃癌等多種腫瘤組織中的磷脂及其相關(guān)物質(zhì)進(jìn)行研究,發(fā)現(xiàn)了磷脂與腫瘤的發(fā)生發(fā)展間的一些關(guān)系。作為潛在的有效腫瘤標(biāo)志物,磷脂目前在胃癌中的研究還比較缺乏。方法采用MALDI-FTICRMS技術(shù),對實(shí)驗(yàn)組5組(對照組、萎縮性胃炎或腸上皮化生組、低級別上皮內(nèi)瘤變組、高級別上皮內(nèi)瘤變組、早期胃癌組每組22例,共110例)血清標(biāo)本進(jìn)行分析并鑒定磷脂分子。通過統(tǒng)計學(xué)方法尋找差異磷脂分子,并進(jìn)行受試者操作特性曲線(ROC)分析。在驗(yàn)證組中(對照組、早期胃癌組每組24例,共48例)驗(yàn)證實(shí)驗(yàn)組差異磷脂分子及其診斷效能。除此之外,對驗(yàn)證組中早期胃癌組患者術(shù)前、術(shù)后血清標(biāo)本中的磷脂分子進(jìn)行鑒定分析,尋找比較術(shù)前、術(shù)后磷脂分子變化。結(jié)果本研究在所有血清中共鑒定19個磷脂分子。實(shí)驗(yàn)組中,8個磷脂分子在不同組間存在差異,包括 PA(...
