縮宮素對(duì)腸道巨噬細(xì)胞極化的調(diào)節(jié)及其治療實(shí)驗(yàn)性結(jié)腸炎的機(jī)制研究

炎癥性腸。↖nflammatory bowel disease,IBD)是一種全球性的腸道慢性炎癥,主要包括克羅恩病和潰瘍性結(jié)腸炎兩種亞型。近20年來(lái)我國(guó)發(fā)病率逐漸增高,已經(jīng)從罕見(jiàn)病過(guò)渡為多見(jiàn)病。炎癥性腸病的發(fā)病機(jī)制目前尚不明確,公認(rèn)的學(xué)說(shuō)認(rèn)為IBD是在環(huán)境因素、遺傳因素等多種因素的共同作用下,腸道免疫系統(tǒng)對(duì)腸道的益生菌反應(yīng)過(guò)強(qiáng)或者腸道菌群紊亂誘發(fā)腸道異常的免疫反應(yīng)造成的。目前臨床上缺乏有效的治療手段,病情反復(fù),遷延不愈,給國(guó)家和個(gè)人帶來(lái)沉重的負(fù)擔(dān)。近些年研究發(fā)現(xiàn)固有免疫的異?赡茉贗BD的發(fā)生和發(fā)展中發(fā)揮著重要作用,而腸道巨噬細(xì)胞可能在其中發(fā)揮著核心作用。腸道的巨噬細(xì)胞主要來(lái)源于骨髓單核細(xì)胞,并根據(jù)腸道局部微環(huán)境的不同,極化為兩種亞型,即經(jīng)典激活和替代激活的巨噬細(xì)胞,也被稱為M1型和M2型的巨噬細(xì)胞。M1型巨噬細(xì)胞對(duì)細(xì)菌內(nèi)毒素反應(yīng)敏感,激活后可以產(chǎn)生和分泌大量的炎癥性細(xì)胞因子,如IL-1β、IL-6、IL-23、TNF-αα等,募集炎細(xì)胞,促進(jìn)局部炎癥反應(yīng)和組織損傷。而M2型巨噬細(xì)胞對(duì)內(nèi)毒素等刺激不敏感,主要分泌IL-10、TGF-β等,抑制炎癥反應(yīng),促進(jìn)組織修復(fù)。越來(lái)越多的研究發(fā)...
