【摘要】：信息技術(shù)的不斷進(jìn)步帶動了醫(yī)學(xué)信息的發(fā)展,也使遠(yuǎn)程醫(yī)療在新世紀(jì)得到了飛速發(fā)展,為解決我國長期以來的“看病難、看病貴”問題提供了新的解決途徑。隨著國家政策的引導(dǎo)以及全國各地遠(yuǎn)程醫(yī)學(xué)中心的建立,遠(yuǎn)程會診量將大幅上升,現(xiàn)有的遠(yuǎn)程會診資源配置很可能不能滿足日益增長的遠(yuǎn)程會診需求。如何合理的對醫(yī)療資源進(jìn)行配置,既能保障遠(yuǎn)程會診的服務(wù)率,又不至于空置和浪費(fèi)診室、專家等優(yōu)質(zhì)資源,將成為醫(yī)院管理中的一個(gè)重要的研究課題。本文以排隊(duì)論為理論依據(jù),首先對遠(yuǎn)程會診流程在理論上建立排隊(duì)網(wǎng)絡(luò)模型,并對所求的主要數(shù)量指標(biāo)進(jìn)行了理論推導(dǎo)。然后利用Simulink仿真平臺對排隊(duì)模型建立仿真模型。最后對鄭州大學(xué)第一附屬醫(yī)院河南省遠(yuǎn)程醫(yī)學(xué)中心的遠(yuǎn)程會診應(yīng)用進(jìn)行實(shí)證研究。根據(jù)獲得的近一年的21295個(gè)遠(yuǎn)程會診數(shù)據(jù),運(yùn)用Minitab和Excel軟件對數(shù)據(jù)進(jìn)行處理后對遠(yuǎn)程會診的會診申請率λ和會診服務(wù)率μ進(jìn)行參數(shù)估計(jì),得出相關(guān)的參數(shù)組合后,利用建立的遠(yuǎn)程會診排隊(duì)仿真模型對其進(jìn)行模擬仿真。通過對仿真結(jié)果的分析,甄別出影響遠(yuǎn)程會診排隊(duì)網(wǎng)絡(luò)擁堵的主要因素,并據(jù)此提出可行的優(yōu)化建議。本文將理論分析與實(shí)證研究結(jié)合,為遠(yuǎn)程會診資源配置的研究提供理論支持,豐富了排隊(duì)論在遠(yuǎn)程會診方面的研究與應(yīng)用。不僅為實(shí)證研究的研究對象提供了具體的優(yōu)化措施,也在理論上為遠(yuǎn)程會診的發(fā)展提供了參考,對遠(yuǎn)程會診的發(fā)展具有一定的推動意義。
[Abstract]:The continuous progress of information technology has led to the development of medical information, but also the rapid development of telemedicine in the new century, in order to solve our country for a long time "difficult to see a doctor." With the guidance of national policy and the establishment of telemedicine centers all over the country, the number of telemedicine consultation will increase greatly. The existing remote consultation resources allocation may not be able to meet the increasing demand for remote consultation. How to rationally configure medical resources can not only guarantee the service rate of remote consultation, but also not empty and waste the diagnosis room. Experts and other high-quality resources will become an important research topic in hospital management. Based on the queuing theory, this paper first establishes the queuing network model for the remote consultation process theoretically. The main quantitative indexes are deduced theoretically, and then the queuing model is established by using the Simulink simulation platform. Finally, the telemedicine center of Henan Province affiliated to the first affiliated Hospital of Zhengzhou University is analyzed. Based on 21295 remote consultation data obtained in one year. After processing the data with Minitab and Excel software, the parameter estimation of consultation application rate 位 and consultation service rate 渭 of remote consultation is carried out, and the related parameter combination is obtained. Through the analysis of the simulation results, the main factors affecting the congestion of the remote consultation queuing network are identified. Based on this, some feasible optimization suggestions are put forward. This paper combines theoretical analysis with empirical research to provide theoretical support for the research of remote consultation resource allocation. It enriches the research and application of queuing theory in remote consultation. It not only provides specific optimization measures for the research object of empirical research, but also provides a reference for the development of remote consultation in theory. It is of great significance to the development of remote consultation.
