【摘要】:目的本論文以知識(shí)考古學(xué)為方法,以研究風(fēng)的概念為切入點(diǎn),采用知識(shí)考古學(xué)話語(yǔ)構(gòu)成分析的方法,考察風(fēng)的概念與中風(fēng)病因病機(jī)理論的關(guān)系。探究?jī)烧邚臐h唐時(shí)期至明清時(shí)期,中風(fēng)病泛發(fā)外風(fēng)說(shuō)至肝風(fēng)內(nèi)動(dòng)說(shuō)的演變,分析中風(fēng)病因病機(jī)理論及其背后的知識(shí)結(jié)構(gòu)體系形成和演變的歷史。方法本論文采用知識(shí)考古學(xué)話語(yǔ)構(gòu)成的分析方法,對(duì)中風(fēng)病病因病機(jī)理論的歷史進(jìn)行考察,通過(guò)對(duì)象的形成、陳述方式、概念的擴(kuò)散、主題策略的選擇及概念間的關(guān)系等內(nèi)容的考察,分析中風(fēng)由泛發(fā)外風(fēng)說(shuō)至肝風(fēng)內(nèi)動(dòng)說(shuō)理論的演變現(xiàn)象。1.考察《黃帝內(nèi)經(jīng)》中"風(fēng)"概念對(duì)象的形成,也就是歷代醫(yī)家對(duì)"風(fēng)"所指范圍的確定,即"泛風(fēng)"指的是什么,"外風(fēng)"指的是什么,"專風(fēng)"指的是什么。對(duì)于"風(fēng)"概念的界定及陳述方式,其分別以什么方式、與哪些概念、以什么次序關(guān)系出現(xiàn)在當(dāng)時(shí)的中醫(yī)理論體系當(dāng)中。2.考察歷代醫(yī)學(xué)文獻(xiàn)中中風(fēng)病因病機(jī)從屬的主題,即中風(fēng)病因病機(jī)是在討論什么問(wèn)題時(shí)出現(xiàn)及其臨床應(yīng)用的改變。3.考察漢唐時(shí)期文獻(xiàn)中,"泛風(fēng)"說(shuō)對(duì)于中風(fēng)病病因病機(jī)的陳述方式,其理論所從屬的主題。4.考察宋元時(shí)期文獻(xiàn)中,外風(fēng)說(shuō)之下產(chǎn)生的肝中外風(fēng)與痰火對(duì)于中風(fēng)病病因病機(jī)的陳述方式,其理論所從屬的主題。5.考察明清時(shí)期文獻(xiàn)中,"專風(fēng)"視角下的肝風(fēng)說(shuō)對(duì)于中風(fēng)病病因病機(jī)的陳述方式,其理論所從屬的主題。結(jié)果漢唐時(shí)期,在"天人相應(yīng)"、"形氣相合"觀念的影響下,以術(shù)數(shù)為學(xué)術(shù)背景、經(jīng)學(xué)影響下的陰陽(yáng)五行成為主流意識(shí)形態(tài)。以《黃帝內(nèi)經(jīng)》為理論源頭,出現(xiàn)了"泛風(fēng)"、外風(fēng)及"風(fēng)",即成為了"風(fēng)"的對(duì)象。"泛風(fēng)"被概括為大自然中感受的多種風(fēng),風(fēng)是彌泛于大自然的,是風(fēng)所呈現(xiàn)出的一種狀態(tài),可以指四季八方皆是風(fēng);外風(fēng)可以理解為自然界流動(dòng)之氣發(fā)生變化成為風(fēng)邪,即六淫之風(fēng)邪,外在的風(fēng)邪侵犯并進(jìn)入人體。在"泛風(fēng)"轉(zhuǎn)變至"專風(fēng)"的過(guò)程中,由于風(fēng)與肝相應(yīng),肝氣儲(chǔ)藏應(yīng)于春的少陽(yáng)之氣。風(fēng)與木是直接對(duì)應(yīng)的關(guān)系,天之木運(yùn)主風(fēng),風(fēng)燥之氣入肝。肝為厥陰風(fēng)木,于是成為了"專風(fēng)"。"泛風(fēng)"說(shuō)對(duì)于中風(fēng)的陳述方式表現(xiàn)在《金匱要略》及《諸病源候論》的內(nèi)容上,中風(fēng)病因病機(jī)以正氣不足,外風(fēng)泛中五臟六腑為主,提出臟腑經(jīng)絡(luò)皆可中風(fēng)。這一時(shí)期有關(guān)中風(fēng)的內(nèi)容繁雜,理論較為模糊不清,尚未有較系統(tǒng)的論述。宋元時(shí)期,戰(zhàn)役的發(fā)生造成熱病的盛行及五運(yùn)六氣在醫(yī)學(xué)界的影響下,外風(fēng)的概念逐漸擴(kuò)展并延伸,出現(xiàn)了肝中外風(fēng)說(shuō)及痰火說(shuō)。盡管宋以后的內(nèi)因論中仍有外風(fēng)理論的闡述,但從理論結(jié)構(gòu)上看,內(nèi)因論的理論結(jié)構(gòu)和診治思路與外因論存在較大的不同。宋元醫(yī)家持內(nèi)外相因的特點(diǎn),強(qiáng)調(diào)內(nèi)因但未完全否認(rèn)外風(fēng)致病,中風(fēng)病因病機(jī)以內(nèi)傷積損為主。明清時(shí)期,在理學(xué)的基礎(chǔ)上提出體用一源,其理論背景導(dǎo)致新學(xué)說(shuō)的產(chǎn)生。"專風(fēng)"視角下的肝風(fēng)成為這個(gè)時(shí)期對(duì)于中風(fēng)的認(rèn)知模式。在非風(fēng)及內(nèi)風(fēng)的新理論出現(xiàn)下,中風(fēng)病因病機(jī)開始以內(nèi)因論為主軸。由此可見(jiàn),中風(fēng)理論隨著風(fēng)的指代對(duì)象不同而變化,其內(nèi)涵亦隨之而改變。結(jié)論1.中醫(yī)"中風(fēng)"的理論內(nèi)涵隨著"風(fēng)"所指代對(duì)象不同而發(fā)生變化。2.伴隨著話語(yǔ)場(chǎng)的改變,不同時(shí)期隨著世界圖示和人體認(rèn)知模式的變化,中風(fēng)病因病機(jī)理論的演變經(jīng)歷了三個(gè)階段:泛發(fā)外風(fēng)、內(nèi)外相因、肝風(fēng)內(nèi)動(dòng)。
