大連地區(qū)HLAR腸球菌的耐藥特點(diǎn)和相關(guān)耐藥基因的研究

【摘要】:目的:通過對大連地區(qū)2014年耐高水平氨基糖苷類抗生素的腸球菌(HLAR)在臨床的檢出率、對抗生素耐藥情況及其耐藥基因的臨床分布特點(diǎn)進(jìn)行分析,為臨床預(yù)防和治療腸球菌感染及后續(xù)相關(guān)研究提供事實(shí)依據(jù)。方法:收集2014年1月-2014年12月大連醫(yī)科大學(xué)附屬第一醫(yī)院患者標(biāo)本中分離出的致病性腸球菌,根據(jù)臨床藥敏結(jié)果篩選出HLAR菌株并采用-80℃低溫冷凍的方法保存。使用LIS系統(tǒng)中的微生物統(tǒng)計(jì)軟件和WHONET 5.6軟件對檢測到的HLAR菌的臨床分布情況和對各類抗生素的耐藥情況等進(jìn)行統(tǒng)計(jì)分析。取出低溫保存的菌種,復(fù)蘇后的細(xì)菌用溶菌酶處理破壞其細(xì)胞壁,使用細(xì)菌基因組提取試劑盒提取出HLAR腸球菌中的遺傳物質(zhì)DNA,再利用聚合酶鏈?zhǔn)椒磻?yīng)和瓊脂糖凝膠電泳法檢測三種常見的氨基糖苷類修飾酶基因:aac(6′)/aph(2″)、aph(3′)2Ⅲ和ant(6)。檢測結(jié)果使用Spss17.0統(tǒng)計(jì)軟件進(jìn)行分析。結(jié)果:2014年從臨床共分離到171株致病腸球菌(無重復(fù)株),從中篩選出101株HLAR菌株,包括64株屎腸球菌,33株糞腸球菌,2株鳥腸球菌,1株鶉雞腸球菌,1株棉籽糖腸球菌。檢測到的101株HLAR腸球菌分布于臨床多個(gè)科室,其中重癥醫(yī)學(xué)科分離到的菌株明顯多于其他科室;從標(biāo)本來源上看,尿液標(biāo)本中分離到的菌株最多,其次為血液標(biāo)本,此外各種分泌物標(biāo)本及膽汁、腹水和膿汁等標(biāo)本中也均檢測出HLAR腸球菌。分析101株HLAR腸球菌的耐藥特性發(fā)現(xiàn):不同種腸球菌其耐藥特性存在差異。屎腸球菌為實(shí)驗(yàn)中檢出率最高的腸球菌,它的耐藥情況最為嚴(yán)峻,對紅霉素耐藥率高達(dá)95.31%,而對環(huán)丙沙星、左旋氧氟沙星和氨芐青霉素等常用抗生素的耐藥率均大于90%。對于上述四種抗生素,糞腸球菌的耐藥情況明顯好于屎腸球菌,但其對喹努普汀/達(dá)福普汀天然耐藥,對四環(huán)素耐藥率也達(dá)到87.87%。兩種腸球菌對常用幾種抗生素的耐藥率明顯不同,經(jīng)χ2檢驗(yàn)其差別具有統(tǒng)計(jì)學(xué)意義,P0.05。實(shí)驗(yàn)中的101株HLAR腸球菌對利奈唑胺和達(dá)托霉素兩種抗生素均為敏感,但檢測到3株耐萬古霉素腸球菌,除1株鶉雞腸球菌對萬古霉素天然耐藥外,另2株均為屎腸球菌。屎腸球菌aac(6′)/aph(2″)、aph(3′)2Ⅲ、ant(6)三種耐藥基因的檢出率分別為87.50%,37.5%和40.6%。糞腸球菌三種基因的檢出率分別為81.82%,24.24%和27.27%,鳥腸球菌等其他種腸球菌未檢測到耐藥基因。實(shí)驗(yàn)也未檢測到同時(shí)具有三種耐藥基因的腸球菌。101株菌株中,aac(6′)/aph(2″)基因的檢出率最高。結(jié)論:臨床HLAR腸球菌感染率較高,多來源于ICU病房和侵入性操作較多的科室。不同種腸球菌耐藥特性不同,屎腸球菌為臨床主要流行的腸球菌種,其耐藥情況最為嚴(yán)峻。aac(6′)/aph(2″)耐藥基因的檢出率最高,是大連地區(qū)引起HLAR腸球菌的主要耐藥基因型。
