攜帶rmtB基因的大腸埃希菌β-內(nèi)酰胺酶基因研究

【摘要】:目的調(diào)查一組攜帶rmtB基因的大腸埃希菌中β-內(nèi)酰胺酶基因與可移動遺傳元件的存在狀況,了解該組菌株之間的親緣關(guān)系。方法收集2016年1-10月湖北醫(yī)藥學(xué)院附屬襄陽市第一人民醫(yī)院住院患者痰液標本分離到的20株攜帶rmtB基因的大腸埃希菌,采用K-B法測定9種抗菌藥物的敏感性,采用聚合酶鏈反應(yīng)(PCR)及序列分析法分析16種β-內(nèi)酰胺酶基因和7種可移動遺傳元件遺傳標記;陽性耐藥基因測序后直接作BLAST比對,耐藥基因檢測結(jié)果作樣本聚類分析(UPGMA法)。結(jié)果 20株攜帶rmtB基因的大腸埃希菌對頭孢類、氨基糖苷類、喹諾酮類均耐藥,但對碳青霉烯類均敏感;20株菌均檢出β-內(nèi)酰胺酶基因及可移動遺傳元件遺傳標記;20株菌共檢出5種β-內(nèi)酰胺酶基因和7種可移動遺傳元件遺傳標記,且陽性率較高;樣本聚類分析顯示20株菌有明顯的聚集性,分A與B二個群,并有4個克隆傳播。結(jié)論本組20株攜帶rmtB基因的大腸埃希菌同時攜帶了β-內(nèi)酰胺酶基因和可移動遺傳元件遺傳標記,是對頭孢類和氨基糖苷類產(chǎn)生耐藥的重要原因;本組菌檢出的4個克隆高度疑似醫(yī)院感染,同一克隆菌株攜帶相同基因。
[Abstract]:Objective to investigate the presence of 尾 -lactamase gene and movable genetic elements in Escherichia coli carrying rmtB gene, and to understand the relationship between these strains. Methods from January to October 2016, 20 strains of Escherichia coli carrying rmtB gene were collected from the sputum samples of the first people's Hospital of Xiangyang affiliated to Hubei Institute of Medicine. The sensitivity of 9 antimicrobial agents was determined by K-B method. Polymerase chain reaction (PCR) and sequence analysis were used to analyze 16 尾 -lactamases genes and 7 kinds of mobile genetic element genetic markers, positive drug resistance genes were sequenced and BLAST was compared directly, and the results of drug resistance gene detection were analyzed by sample cluster analysis (UPGMA method). Results 20 strains of Escherichia coli carrying rmtB gene were resistant to cephalosporins, aminoglycosides and quinolones. However, 尾 -lactamase genes were detected in all 20 strains sensitive to carbapenes, and 5 尾 -lactamases genes and 7 transportable genetic element genetic markers were detected in 20 strains with movable genetic elements, and the positive rate was higher. The sample cluster analysis showed that 20 strains had obvious aggregation, which were divided into A and B groups, and 4 clones were transmitted. Conclusion the 20 strains of Escherichia coli carrying rmtB gene carry both 尾 -lactamase gene and mobile genetic element genetic markers, which is an important cause of drug resistance to cephalosporins and aminoglycosides. Four clones were highly suspected of nosocomial infection, and the same cloned strain carried the same gene.
