【摘要】:目的對(duì)世界范圍內(nèi)血管緊張素轉(zhuǎn)換酶基因多態(tài)性(ACE I/D)與腦小血管病相關(guān)性的研究進(jìn)行Meta分析,明確ACE I/D基因多態(tài)性與腦小血管病發(fā)病是否相關(guān)。方法全面檢索有關(guān)的血管緊張素轉(zhuǎn)換酶基因多態(tài)性和腦小血管病相關(guān)性的文獻(xiàn),制定統(tǒng)一的檢索策略,檢索數(shù)據(jù)庫(kù)包括中國(guó)知網(wǎng)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)光盤數(shù)據(jù)庫(kù)、醫(yī)知網(wǎng)、萬方、維普數(shù)據(jù)庫(kù),Pubmed、Google以及參考文獻(xiàn)檢索等手段。將檢索到的文獻(xiàn)按納入排除標(biāo)準(zhǔn)進(jìn)行篩選,再依據(jù)制定的質(zhì)量評(píng)價(jià)表,對(duì)篩選出符合本研究要求的文獻(xiàn)進(jìn)行質(zhì)量評(píng)價(jià)。統(tǒng)計(jì)學(xué)分析采用Cochrane協(xié)作網(wǎng)提供的Rev Man5.3軟件,對(duì)二分類變量使用Mantel-Haenszel法進(jìn)行合并分析,檢驗(yàn)水準(zhǔn)為α=0.05;用Q值和I2檢查各獨(dú)立研究的異質(zhì)性,異質(zhì)性檢驗(yàn)水準(zhǔn)設(shè)為α=0.1;潛在的發(fā)表偏倚用漏斗圖評(píng)估。結(jié)果依據(jù)制定的統(tǒng)一檢索策略,共檢索出相關(guān)文獻(xiàn)377篇,再依據(jù)制定的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn),最終納入符合條件的獨(dú)立研究共26篇,總共7094例受試對(duì)象。經(jīng)過質(zhì)量評(píng)價(jià),11篇文獻(xiàn)為高質(zhì)量研究,15篇文獻(xiàn)為中等質(zhì)量研究。經(jīng)分析結(jié)果顯示:血管緊張素轉(zhuǎn)換酶ACE(I/D)基因多態(tài)性與腦小血管病在顯性模型下(DD+IDvs I I):OR=1.30,95%CI=(1.11,1.52),P=0.001;隱性模型(DDvs ID+II):OR=1.44,95%CI=(1.19,1.75),P=0.0002;共顯性模型(DDvs II):OR=1.59,95%CI=(1.26,1.99),P0.0001以及(I Dvs II):OR=1.16,95%CI=(1.02,1.32),P=0.02;等位基因比較(Dvs I):OR=1.30,95%CI=(1.15,1.48),P0.0001。再分別對(duì)腦小血管病的不同臨床分型和不同種族進(jìn)行分析,多數(shù)遺傳模型下可得到類似結(jié)果。結(jié)論通過上述研究結(jié)果說明血管緊張素轉(zhuǎn)換酶(ACE I/D)基因多態(tài)性與腦小血管病的發(fā)病相關(guān),DD基因型和D等位基因可以增加該疾病的患病風(fēng)險(xiǎn)。在亞洲人和高加索人中ACE I/D基因多態(tài)性與腦小血管病發(fā)生顯著相關(guān),該基因多態(tài)性與腔隙性腦梗死發(fā)生顯著相關(guān)。
[Abstract]:Objective to investigate the association of ACE I / D polymorphism with cerebral microangiopathy in the world, and to determine whether ACE I / D polymorphism is related to the pathogenesis of cerebral microangiopathy. Methods A comprehensive search was carried out on the relationship between angiotensin converting enzyme gene polymorphism and cerebral microangiopathy, and a unified search strategy was established. The retrieval databases included China knowledge Network, Chinese Biomedical Literature CD-ROM database and Medical knowledge Network. Wanfang, Wiper Database, PubmedCon Google and reference search and other means. The retrieved documents were screened according to the exclusion criteria, and then the quality evaluation of the selected documents according to the quality evaluation table was carried out. The statistical analysis was performed with the software Rev Man5.3 provided by Cochrane Cooperative Network, and the Mantel-Haenszel method was used to combine the two classified variables. The test level was 偽 -0.05.The heterogeneity of each independent study was examined by Q and I2. The heterogeneity test level was set at 偽 -0. 1. The potential publication bias was evaluated by funnel graph. Results according to the unified retrieval strategy, a total of 377 articles were retrieved, and 26 independent studies were included according to the inclusion criteria and exclusion criteria.
