甲狀腺乳頭狀癌組織中TSHR、NIS基因啟動(dòng)子區(qū)甲基化狀態(tài)與臨床病理特征的關(guān)系

【摘要】:目的探討甲狀腺乳頭狀癌(PTC)組織中促甲狀腺激素受體(TSHR)與鈉碘轉(zhuǎn)運(yùn)體(NIS)基因啟動(dòng)子區(qū)甲基化狀態(tài)與臨床病理特征的關(guān)系。方法收集80例PTC患者臨床資料及手術(shù)病理標(biāo)本石蠟切片,采用實(shí)時(shí)熒光定量甲基化特異性PCR(q MSP)法檢測(cè)TSHR和NIS啟動(dòng)子區(qū)甲基化狀態(tài),分析二者與患者臨床病理特征的關(guān)系。結(jié)果 PTC組織中TSHR與NIS基因啟動(dòng)子區(qū)甲基化發(fā)生率分別為71.25%、32.50%,癌旁正常組織中分別為8.75%、6.25%,兩者比較差異有統(tǒng)計(jì)學(xué)意義(P均0.05);不同年齡、性別、腫瘤直徑PTC患者癌組織中TSHR與NIS基因啟動(dòng)子區(qū)甲基化情況比較無明顯統(tǒng)計(jì)學(xué)差異(P均0.05),病灶多發(fā)、腺外侵犯、有頸部淋巴結(jié)轉(zhuǎn)移、臨床分期Ⅲ~Ⅳ期PTC患者癌組織中TSHR與NIS甲基化程度高于病灶單發(fā)、局限腺內(nèi)、無頸部淋巴結(jié)轉(zhuǎn)移及臨床分期Ⅰ~Ⅱ期者(P均0.05)。Spearman相關(guān)分析顯示,PTC組織中TSHR與NIS基因啟動(dòng)子區(qū)甲基化呈正相關(guān)(r=0.532,P0.05)。結(jié)論 PTC組織中TSHR與NIS基因啟動(dòng)子區(qū)甲基化現(xiàn)象普遍存在,與病灶多發(fā)、頸部淋巴結(jié)轉(zhuǎn)移、腺外侵犯、臨床分期關(guān)系密切,兩個(gè)基因啟動(dòng)子區(qū)域甲基化協(xié)同作用可能是引起腫瘤發(fā)展及轉(zhuǎn)移的相關(guān)機(jī)制之一。
[Abstract]:Objective to investigate the relationship between the methylation status of thyroid stimulating hormone receptor (TSHR) and sodium iodide transporter (NIS) gene promoter and clinicopathological features in papillary thyroid carcinoma (PTC). Methods the clinical data and paraffin sections of 80 patients with PTC were collected. The methylation status of TSHR and NIS promoter region was detected by real-time fluorescence quantitative methylation specific PCR(q (PCR(q) method, and the relationship between them and the clinicopathological features of the patients was analyzed. Results the incidences of methylation in promoter region of TSHR and NIS gene in PTC tissues were 71.25 and 32.50, respectively, and those in adjacent normal tissues were 8.75 and 6.25.The differences were statistically significant (P < 0.05). There was no significant difference in methylation between the promoter region of TSHR and NIS gene in patients with tumor diameter PTC (P < 0.05), with multiple lesions, extraglandular invasion and cervical lymph node metastasis.
