攜帶wcaG毒力基因ST19產(chǎn)酸克雷伯菌致白血病患者死亡的分析

【摘要】:目的了解1例高黏液表型(HMV)產(chǎn)酸克雷伯菌致白血病患者的死亡原因。方法采用Vitek 2Compact全自動微生物儀進(jìn)行菌株鑒定及藥敏試驗,黏液絲試驗檢測HMV表型,PCR方法及基因測序檢測主要的高毒力莢膜血清型基因(K1、K2、K5、K20、K54、K57)和毒力基因(rmpA、wcaG、allS、kfu、aerobactin、mrkD、fimH、uge、wabG、cf29a),多位點序列分析(MLST)對該菌株進(jìn)行分子分型。結(jié)果白血病患者血液及肺組織分離株均為產(chǎn)酸克雷伯菌,MLST分型為同一型ST19,該菌株對所測藥物均敏感,具有高黏液表型,只檢測到wcaG毒力基因,其他毒力基因和所測莢膜血清型基因均陰性。結(jié)論攜帶wcaG毒力基因是ST19產(chǎn)酸克雷伯菌致白血病患者死亡的主要原因,應(yīng)引起臨床關(guān)注。
[Abstract]:Objective to investigate the causes of death in a case of leukemia caused by Klebsiella acidogenic bacteria with high mucus phenotype.Methods the strain identification and drug sensitivity test were carried out by Vitek 2Compact automatic microorganism instrument.Detection of HMV phenotype by Myxofilament Test and Gene sequencing Analysis of the main highly virulent serotype gene K1K2OK2K5OK20K20K54K57) and virulence gene rmpAgalus wcagall Skfuall aerobactinia mrkDfimHugeugeugewabGcf29aan (multilocus sequence analysis) were used for molecular typing of the strain.Results the blood and lung tissues of leukemia patients were classified into the same type ST19 by MLST of Klebsiella acidogenic strain. The strain was sensitive to the drugs tested and had high mucus phenotype. Only wcaG virulence gene was detected.All other virulence genes and serotype genes were negative.Conclusion carrying wcaG virulence gene is the main cause of death in leukemia patients caused by Klebsiella ST19.
