【摘要】：補(bǔ)償患者高值藥品費(fèi)用是大病醫(yī)療保障的重要途徑。我國(guó)香港地區(qū)建立了藥物安全網(wǎng),在提高高值藥品可及性、減輕患者疾病負(fù)擔(dān)等方面取得了顯著成效。本文系統(tǒng)梳理了香港地區(qū)藥物安全網(wǎng)的歷史沿革、藥品納入流程、藥品及患者申請(qǐng)條件、發(fā)展現(xiàn)狀以及特點(diǎn)。指出我國(guó)內(nèi)地雖已逐步通過藥品談判建立保障大病的高值藥品目錄,但機(jī)制尚不成熟。應(yīng)借鑒香港地區(qū)藥物安全網(wǎng),建立高值藥品目錄的準(zhǔn)入與退出機(jī)制,實(shí)現(xiàn)高值藥品目錄與基本醫(yī)療保險(xiǎn)目錄、新農(nóng)合基本用藥目錄的銜接。同時(shí)建立多渠道、社會(huì)化的籌資機(jī)制,擴(kuò)大高值藥品保障范圍。最后堅(jiān)持買贈(zèng)結(jié)合的談判原則,改革單一的定比例補(bǔ)償方式,以進(jìn)一步加強(qiáng)大病醫(yī)療保障。
[Abstract]:It is an important way to compensate the high value drug cost of patients for medical security for serious diseases. A drug safety net has been established in Hong Kong, China, which has achieved remarkable results in improving the accessibility of high-value drugs and reducing the burden of diseases on patients. This paper systematically combs the historical evolution, drug inclusion process, drug and patient application conditions, development status and characteristics of drug safety network in Hong Kong. It is pointed out that although the mainland of China has gradually established a catalogue of high value drugs to guarantee serious diseases through drug negotiation, the mechanism is not yet mature. We should draw lessons from the drug safety net in Hong Kong, establish the access and withdrawal mechanism of the catalogue of high value drugs, and realize the connection between the catalogue of high value drugs and the catalogue of basic medical insurance and the catalogue of basic drugs used by NCMS. At the same time, we will establish a multi-channel and socialized financing mechanism to expand the scope of high-value drug security. Finally, we should adhere to the negotiation principle of the combination of purchase and gift, and reform the single proportional compensation method in order to further strengthen the medical security of serious illness.
