城鎮(zhèn)基本醫(yī)療保險(xiǎn)中逆向選擇的檢驗(yàn)

【摘要】：本文利用城鎮(zhèn)居民基本醫(yī)療保險(xiǎn)試點(diǎn)評(píng)估調(diào)查的數(shù)據(jù),實(shí)證檢驗(yàn)了我國(guó)城鎮(zhèn)居民和職工基本醫(yī)療保險(xiǎn)中的逆向選擇問題。本文的實(shí)證結(jié)果證實(shí)了逆向選擇的存在:在未被城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)覆蓋的城鎮(zhèn)人群中,健康狀況較差的個(gè)體更傾向于參加城鎮(zhèn)居民基本醫(yī)療;城鎮(zhèn)居民基本醫(yī)療保險(xiǎn)的參保者接受門診和住院服務(wù)利用率的概率更高;保險(xiǎn)對(duì)住院服務(wù)利用率的影響大于對(duì)門診的影響。另一方面,我們還發(fā)現(xiàn)在已被城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)覆蓋的人群中,健康狀況較差的職工也更傾向于購(gòu)買補(bǔ)充商業(yè)醫(yī)保,但是健康狀況最差的個(gè)體購(gòu)買補(bǔ)充商業(yè)醫(yī)保的概率最低,而購(gòu)買商業(yè)醫(yī)保后住院率顯著增加。
[Abstract]:Based on the data of pilot evaluation survey of basic medical insurance for urban residents, this paper empirically tests the problem of adverse selection in basic medical insurance for urban residents and workers in China. The empirical results of this paper confirm the existence of adverse selection: in the urban population not covered by the basic medical insurance for urban workers, the individuals with poor health status are more likely to participate in the basic medical care of urban residents; The probability of receiving outpatient and inpatient service utilization rate of urban residents' basic medical insurance is higher, and the influence of insurance on the utilization rate of inpatient service is greater than that on outpatient service. On the other hand, we also find that among the people already covered by the basic medical insurance for urban workers, workers with poor health status are also more likely to buy supplementary commercial medical insurance. However, individuals with the worst health conditions were the lowest likely to buy supplementary commercial health insurance, while the hospitalization rate increased significantly after buying commercial health insurance.
