【摘要】：長(zhǎng)期護(hù)理保險(xiǎn)制度是我國(guó)老齡化背景下應(yīng)對(duì)家庭照護(hù)資源不足、社會(huì)保障支持欠缺的重要政策。該制度在我國(guó)尚處起步階段,需要進(jìn)一步總結(jié)試點(diǎn)地市的實(shí)踐經(jīng)驗(yàn)。通過對(duì)青島市、濰坊市和聊城市6個(gè)定點(diǎn)專護(hù)機(jī)構(gòu)16位被訪者進(jìn)行實(shí)地訪談,探討醫(yī)療專護(hù)制度的運(yùn)行模式特征。結(jié)果表明,定點(diǎn)專護(hù)機(jī)構(gòu)以基層單位為主;除青島市外,試點(diǎn)地市的目標(biāo)人口僅覆蓋職工醫(yī)療保險(xiǎn),且均存在戶籍限制;服務(wù)項(xiàng)目排除生活護(hù)理,抑制了照護(hù)服務(wù)需求;生活護(hù)理管理模式影響專護(hù)服務(wù)質(zhì)量;結(jié)算標(biāo)準(zhǔn)比例與不斷變動(dòng)的醫(yī)療服務(wù)價(jià)格不吻合。探討生活護(hù)理員的管理模式、籌資機(jī)制的頂層設(shè)計(jì)以及政府多部門協(xié)同是解決上述問題,確保長(zhǎng)期護(hù)理保險(xiǎn)制度可持續(xù)運(yùn)行的根本點(diǎn)。
[Abstract]:The long-term nursing insurance system is an important policy to deal with the shortage of family care resources and the lack of social security support under the background of aging in China. This system is still in its infancy in our country, so it needs to summarize the practical experience of the pilot city. Through on-the-spot interviews with 16 interviewees from 6 fixed-point special care institutions in Qingdao, Weifang and Liaocheng, this paper discusses the characteristics of the operation mode of medical special care system. The results show that the designated special care institutions are mainly grass-roots units, except Qingdao, the target population of pilot areas only covers medical insurance for workers, and all of them have household registration restrictions, and the service items exclude life care and restrain the demand for care services. The management mode of life nursing affects the quality of special care service, and the proportion of settlement standard is not consistent with the changing price of medical service. Probing into the management mode of life nurses, the top-level design of financing mechanism and the coordination of government departments are the root points for solving the above problems and ensuring the sustainable operation of the long-term nursing insurance system.
