【摘要】： 面對(duì)農(nóng)民衛(wèi)生費(fèi)用的不斷攀升,農(nóng)村衛(wèi)生機(jī)構(gòu)服務(wù)質(zhì)量低下,因病致貧返貧現(xiàn)象的不斷增加,面對(duì)全面建設(shè)小康社會(huì)、和諧社會(huì)的宏偉目標(biāo),如何建立起一個(gè)人人可及而又有效的新型農(nóng)村合作醫(yī)療制度,解決農(nóng)民就醫(yī)難、就醫(yī)貴的問(wèn)題已成為理論界和實(shí)踐界的當(dāng)務(wù)之急。 建立新型農(nóng)村合作醫(yī)療制度的最大難題在于合作醫(yī)療基金的籌集。穩(wěn)定而具有一定規(guī)模的合作醫(yī)療基金,是新型農(nóng)村合作醫(yī)療制度順利開(kāi)展的基礎(chǔ)和保證,而科學(xué)合理的合作醫(yī)療基金籌集機(jī)制恰恰是保證農(nóng)村合作醫(yī)療基金穩(wěn)定持續(xù)的關(guān)鍵,也是開(kāi)展和推廣新型農(nóng)村合作醫(yī)療制度的關(guān)鍵。 本文在系統(tǒng)深入總結(jié)國(guó)內(nèi)外相關(guān)文獻(xiàn)的基礎(chǔ)上,介紹了農(nóng)村合作醫(yī)療的基礎(chǔ)理論,回顧了我國(guó)農(nóng)村合作醫(yī)療籌資機(jī)制的變遷過(guò)程,重點(diǎn)分析了山西省新型農(nóng)村合作醫(yī)療的籌資現(xiàn)狀,包括籌資情況、籌資成本、資金統(tǒng)籌層次,從不同角度深入分析資金籌集的影響因素,以期為新型農(nóng)村合作醫(yī)療制度的健康運(yùn)行奠定基礎(chǔ)。通過(guò)對(duì)國(guó)外農(nóng)村醫(yī)療保障制度有益經(jīng)驗(yàn)的借鑒,針對(duì)性地提出了包括籌資原則、籌資方式、籌資渠道等在內(nèi)的一系列政策建議,對(duì)于構(gòu)建新型農(nóng)村合作醫(yī)療制度、改革新型農(nóng)村合作醫(yī)療籌資機(jī)制具有重要的理論意義和現(xiàn)實(shí)意義。
[Abstract]:Facing the rising of farmers' health expenses, the low service quality of rural health institutions, the increasing phenomenon of poverty and returning to poverty caused by illness, and the grand goal of building a well-off society in an all-round way and harmonious society, How to establish a new rural cooperative medical system, which is accessible and effective to everyone, and how to solve the problem that it is difficult for farmers to seek medical treatment and how to seek medical treatment have become an urgent matter for the theoretical and practical circles. The biggest difficulty in establishing the new rural cooperative medical system lies in the raising of the cooperative medical fund. A stable cooperative medical fund with a certain scale is the basis and guarantee for the smooth development of the new rural cooperative medical system, and the scientific and reasonable mechanism of raising the cooperative medical fund is the key to ensure the stability and sustainability of the rural cooperative medical fund. It is also the key to develop and popularize the new rural cooperative medical system. On the basis of systematical and in-depth summary of relevant literature at home and abroad, this paper introduces the basic theory of rural cooperative medical system, and reviews the changing process of financing mechanism of rural cooperative medical system in China. The paper mainly analyzes the current situation of the new rural cooperative medical system in Shanxi Province, including the financing situation, the financing cost, the overall planning level of the fund, and analyzes the influence factors of the fund raising from different angles. In order to lay a foundation for the healthy operation of the new rural cooperative medical system. Based on the beneficial experience of the rural medical security system abroad, this paper puts forward a series of policy recommendations including the principle of financing, the way of raising funds, the channel of financing, and so on, which is helpful to the construction of the new rural cooperative medical system. The reform of the new rural cooperative medical financing mechanism has important theoretical and practical significance.
