【摘要】：新型農(nóng)村合作醫(yī)療(以下簡稱為新農(nóng)合)從2003年開始在我省展開試點，截止2011年年底全省新農(nóng)合參合人數(shù)已經(jīng)超過2100萬人，，參合率超過98%，累計受益人次達2480萬，累計補償資金近20億元。 太原市在2003年開始實施新農(nóng)合，陽曲縣、婁煩縣進入首批國家級試點。2005年，尖草坪區(qū)、迎澤區(qū)、杏花嶺區(qū)、萬柏林區(qū)又成功納入省級試點。到2006年，太原市10縣（市、區(qū)）已全部啟動新農(nóng)合，該市近幾年在農(nóng)村合作醫(yī)療方面得到了長足的發(fā)展。但是不可否認，在新農(nóng)合高速發(fā)展的情況下依然存在因病返貧、因病致貧的現(xiàn)象。究其原應(yīng)主要是管理制度的不完善，籌資水平和補償水平不合理、抗風(fēng)險能力弱，這些問題同時會削弱新農(nóng)合可持續(xù)發(fā)展的動力。 本文首先通過對太原市新農(nóng)合現(xiàn)有制度的闡述以及對其運行情況的介紹，從總體上把握太原市新農(nóng)合制度，并對制度運行的經(jīng)驗進行分析總結(jié)，肯定了該市為進一步促進新農(nóng)合可持續(xù)發(fā)展而做出的努力和成績，同時也相應(yīng)的提出了影響制度可持續(xù)發(fā)展的一些不合理因素。其次，文章詳細的介紹了傳統(tǒng)農(nóng)村合作醫(yī)療從萌芽到衰落的發(fā)展歷程，對其最終失敗的原因進行深入的探究，并且引入“江陰模式”和“洛陽模式”這兩種成功模式，通過對比，得出一些針對太原市特別的啟示。 文章最后從政府角色的定位、制度完善、醫(yī)療以及經(jīng)濟等方面對太原市的新農(nóng)合可持續(xù)發(fā)展提出一些建議，并對制度的發(fā)展趨勢進行預(yù)測，著重強調(diào)建立多層次醫(yī)療網(wǎng)絡(luò)的重要性，逐步縮小城鄉(xiāng)醫(yī)療差距，并最終實現(xiàn)一體化，促進本地區(qū)新型農(nóng)村合作醫(yī)療的可持續(xù)發(fā)展。
[Abstract]:The new rural cooperative medical system (hereinafter referred to as NCMS) has been piloting in our province since 2003. By the end of 2011, the number of NCMS participants in the province had exceeded 21 million, the participation rate was over 98, and the cumulative beneficiaries reached 24.8 million. The accumulative total compensation fund is nearly 2 billion yuan. In 2003, Taiyuan began to implement the new agricultural cooperation, Yangqu County, Loufan County into the first batch of national pilot. In 2005, Jiancaoping District, Yingze District, Xinghualing District, Wan Berlin District were successfully incorporated into the provincial pilot projects. By 2006, 10 counties (cities, districts) of Taiyuan City have all started the New Rural Cooperative Medical system (NCMS), and the city has made great progress in rural cooperative medical care in recent years. However, it is undeniable that under the condition of rapid development of NCMS, there still exists the phenomenon of poverty due to illness and poverty caused by illness. The main reason is that the management system is not perfect, the level of financing and compensation is unreasonable, and the ability to resist risks is weak. These problems will weaken the motive force of sustainable development of NCMS at the same time. First of all, through the exposition of the existing system of the new agricultural cooperation in Taiyuan and the introduction of its operation, this paper grasps the new agricultural cooperation system of Taiyuan on the whole, and analyzes and summarizes the experience of the system operation. The efforts and achievements made by the city to further promote the sustainable development of NCMS are affirmed, and some unreasonable factors affecting the sustainable development of the system are put forward accordingly. Secondly, the article introduces the development process of traditional rural cooperative medical system from germination to decline in detail, probes into the causes of its ultimate failure, and introduces "Jiangyin mode" and "Luoyang mode" as two successful models. Through comparison, we get some special inspiration for Taiyuan. Finally, the paper puts forward some suggestions on the sustainable development of the new agricultural cooperation in Taiyuan from the following aspects: the role of government, the perfection of the system, the medical treatment and the economy, and forecasts the trend of the development of the system. Emphasis is placed on the importance of establishing a multi-level medical network, gradually narrowing the gap between urban and rural medical care, and finally realizing the integration, so as to promote the sustainable development of the new rural cooperative medical system in the region.
