我國(guó)老年長(zhǎng)期照護(hù)保險(xiǎn)籌資、補(bǔ)償水平優(yōu)化設(shè)計(jì)研究——兼論老年照護(hù)保險(xiǎn)框架設(shè)定

【摘要】:雖然我國(guó)部分地區(qū)開(kāi)展了老年長(zhǎng)期照護(hù)保險(xiǎn)試點(diǎn)工作,但是,如何確定科學(xué)的籌資、補(bǔ)償水平已成為阻礙各地老年長(zhǎng)期照護(hù)保險(xiǎn)試點(diǎn)的核心問(wèn)題,也是國(guó)家層面長(zhǎng)照制度探索的核心難題。在老年失能人口預(yù)測(cè)、老年失能人口照護(hù)服務(wù)需求評(píng)估基礎(chǔ)上,結(jié)合老年長(zhǎng)期社會(huì)照護(hù)保險(xiǎn)框架設(shè)計(jì),包括照護(hù)目標(biāo)人群、報(bào)銷比例、政府個(gè)人分擔(dān)比例、長(zhǎng)時(shí)期年度均衡等,綜合、均衡測(cè)算籌資水平和待遇水平。結(jié)果發(fā)現(xiàn),當(dāng)政府與個(gè)人籌資比例為5∶5、補(bǔ)償報(bào)銷比例為90%、保障重度失能老年人基本照護(hù)服務(wù)需求時(shí),個(gè)人繳費(fèi)占可支配收入的均衡比例區(qū)間為0.12%~0.33%,財(cái)政補(bǔ)貼占財(cái)政支出的均衡比例區(qū)間為0.17%~0.47%;即使將常規(guī)失能老人都進(jìn)行保障,相應(yīng)的均衡比例區(qū)間分別為0.24%~0.68%,和0.34%~0.96%,方案負(fù)擔(dān)較輕、制度可持續(xù)性強(qiáng)。在研究結(jié)論基礎(chǔ)上,進(jìn)一步提出了整合照護(hù)資源、完善照護(hù)保險(xiǎn)成本控制等建議。
[Abstract]:Although some regions of our country have carried out the pilot work of long-term care insurance for the elderly, how to determine the scientific financing and the compensation level has become the core problem that hinders the pilot projects of the long-term care insurance for the elderly in various places. Also is the national level long-term light system exploration core difficult problem. On the basis of the prediction of the elderly disabled population and the assessment of the care needs of the elderly disabled population, combined with the design of the long-term social care insurance framework for the elderly, including the care target population, the reimbursement ratio, the government individual share ratio, the long-term annual equilibrium, etc. Comprehensive, balanced measurement of financing level and treatment level. The results show that when the ratio of government to individual financing is 5: 5, and the proportion of compensation and reimbursement is 90, the demand for basic care services for severely disabled elderly people is guaranteed. The equilibrium ratio of individual contributions to disposable income is 0.12 / 0.33, and the equilibrium ratio of financial subsidies to financial expenditure is 0.17 and 0.47; even if the elderly with conventional disability are protected, the corresponding equilibrium ratio ranges are 0.240.68 and 0.34 / 0.96.The plan is less burdensome. Strong institutional sustainability. On the basis of the conclusion, the author puts forward some suggestions such as integrating care resources and perfecting the cost control of care insurance.
