【摘要】：農(nóng)村老齡化問題已成為一個(gè)廣受社會關(guān)注的問題，這個(gè)問題不僅直接影響著我國的經(jīng)濟(jì)、政治、與文化因素，也給農(nóng)村的養(yǎng)老體系提出了巨大的挑戰(zhàn)，這個(gè)問題已經(jīng)成為政府難以回避的重大問題。近年來，醫(yī)療費(fèi)用呈現(xiàn)出逐年上升的趨勢，農(nóng)民“看不起病，吃不起藥”的現(xiàn)象非常普遍，很多農(nóng)民由于經(jīng)濟(jì)原因的影響均不愿入院接受治療，農(nóng)民缺乏醫(yī)療的保障，，看病支出均是自己負(fù)擔(dān)，這就給家庭帶來了巨大的經(jīng)濟(jì)壓力，與城市居民相比而言，農(nóng)民也面臨著更多的疾病隱患，新型農(nóng)村合作醫(yī)療保險(xiǎn)制度正是基于此而產(chǎn)生，該種制度是由政府指導(dǎo)、農(nóng)民自愿參與，政府、集體、個(gè)人共同籌資的一種醫(yī)療互助制度。雖然新型農(nóng)村合作醫(yī)療保險(xiǎn)制度在衡陽縣已經(jīng)實(shí)施了一段時(shí)間，但是依然難以為老年人群體提供全方位的保障。 由于各種因素的影響，自我養(yǎng)老與家庭養(yǎng)老模式還存在著各種不足，因此，社會保險(xiǎn)途徑也會是解決農(nóng)村老年人養(yǎng)老問題的主要途徑，為了完善新型農(nóng)村合作醫(yī)療保險(xiǎn)制度，需要明確政府責(zé)任，加大扶植力度、加強(qiáng)監(jiān)督與管理，促進(jìn)保險(xiǎn)基金的增值、加強(qiáng)宣傳，提升農(nóng)民尤其是老年人的參保意識、完善法律制度，營造出制度環(huán)境、轉(zhuǎn)變農(nóng)村經(jīng)濟(jì)發(fā)展模式，促進(jìn)農(nóng)村經(jīng)濟(jì)的良性循環(huán)，本文正是基于此來分析新農(nóng)合下農(nóng)村老人合作醫(yī)療制度的完善措施。 本文分五章闡述了新農(nóng)合下農(nóng)村老人合作醫(yī)療制度的完善方式，第一章深入的介紹了國內(nèi)外研究現(xiàn)狀與選題意義，得出我國現(xiàn)行的醫(yī)療保險(xiǎn)體系在補(bǔ)償比例、制度設(shè)計(jì)、籌集方式、資金規(guī)模和管理主體等方面都存在著一定的差異，城鄉(xiāng)之間存在著較大的差別，在基金收入與待遇方面，城鎮(zhèn)居民醫(yī)療保險(xiǎn)是新型農(nóng)村合作醫(yī)療人均籌資額的十倍左右，整體的制度有待進(jìn)一步完善。 第二章分析了新型農(nóng)村合作醫(yī)療制度基本原則以及新型農(nóng)村合作醫(yī)療制度的相關(guān)理論，這為本文的研究奠定了堅(jiān)實(shí)的理論技術(shù)。 第三章分析了新農(nóng)合下農(nóng)村老人合作醫(yī)療制度中存在的問題，得出，這些問題有些屬于制度設(shè)計(jì)的范疇，有些屬于政策制定的范疇，有些屬于配套措施的問題，這些問題均嚴(yán)重影響了農(nóng)村老人合作醫(yī)療制度的正常執(zhí)行。 第四章采用問卷調(diào)查的形式分析了衡陽縣新型農(nóng)村合作醫(yī)療制度的執(zhí)行情況，得出無論從經(jīng)濟(jì)水平還是從健康狀況來分析，農(nóng)村老人均稱為社會弱勢群體，其中，疾病是影響農(nóng)村老年人生活質(zhì)量的主要因素，農(nóng)村老年人在患病后，在就診機(jī)構(gòu)的選擇上主要為村衛(wèi)生室、個(gè)體診所與鄉(xiāng)鎮(zhèn)衛(wèi)生院，雖然中央與地方上增加了對衛(wèi)生院的資金投入，但是地方財(cái)政往往難以及時(shí)的落實(shí)，這就導(dǎo)致農(nóng)村衛(wèi)生院的建設(shè)工作滯后，而從總體而言，新農(nóng)合農(nóng)村老人合作醫(yī)療制度還存在種種問題。 第五章是本文的核心，主要根據(jù)新農(nóng)合下農(nóng)村老人合作醫(yī)療制度中存在的問題提出相應(yīng)的解決措施。 本文的研究得出，由于各種因素的影響，自我養(yǎng)老與家庭養(yǎng)老模式還存在著各種不足，因此，社會保險(xiǎn)途徑也會是解決農(nóng)村老年人養(yǎng)老問題的主要途徑，為了完善新型農(nóng)村合作醫(yī)療保險(xiǎn)制度，需要明確政府責(zé)任，加大扶植力度、加強(qiáng)監(jiān)督與管理，促進(jìn)保險(xiǎn)基金的增值、加強(qiáng)宣傳，提升農(nóng)民尤其是老年人的參保意識、完善法律制度，營造出制度環(huán)境、轉(zhuǎn)變農(nóng)村經(jīng)濟(jì)發(fā)展模式，促進(jìn)農(nóng)村經(jīng)濟(jì)的良性循環(huán)。
[Abstract]:The problem of rural aging has become a social concern , which not only directly affects the economic , political and cultural factors of our country , but also poses a great challenge to the old - age system in the countryside . In recent years , the medical expense has become a major problem that the government has difficulty to avoid . In recent years , the farmers are not willing to accept treatment because of the economic reasons . The new rural cooperative medical insurance system is a kind of medical mutual aid system which is led by the government and the farmers are voluntarily involved , the government , the collective and the individual . Although the new rural cooperative medical insurance system has been implemented in HengYang county for some time , it is still difficult to provide all - round guarantee for the elderly population .

Because of the influence of various factors , the self - supporting and the family pension pattern still exist , so the social insurance way is the main way to solve the old - age pension problem in the rural areas . In order to perfect the new rural cooperative medical insurance system , it is necessary to clarify the government responsibility , strengthen the promotion , strengthen the supervision and management , promote the value - added of the insurance fund , build up the system environment , transform the rural economic development mode , and promote the virtuous circle of the rural economy .

In the first chapter , the author gives a detailed introduction to the current situation of medical insurance system in the countryside and the significance of choosing the subject , and concludes that the present medical insurance system has some differences in the proportion of compensation , the system design , the way of raising funds , the scale of funds and the subject of management .

The second chapter analyzes the basic principles of the new rural cooperative medical system and the relevant theories of the new rural cooperative medical system , which lays a solid theoretical technique for the study .

The third chapter analyzes the problems existing in the cooperative medical system of the rural old people under the new agriculture , and concludes that some of these problems belong to the scope of the system design , some belong to the category of the policy formulation and some problems which belong to the matching measures , which seriously affect the normal execution of the rural elderly cooperative medical system .

The fourth chapter analyzes the implementation of the new rural cooperative medical system in HengYang county by means of questionnaire , and concludes that the rural old people are called the social vulnerable groups , whether from the economic level or the health status , among which the disease is the main factor affecting the quality of life of the rural elderly .

The fifth chapter is the core of this paper , and puts forward the corresponding measures according to the problems existing in the cooperative medical system of rural old people under the new agriculture .

In order to perfect the new rural cooperative medical insurance system , it is necessary to clarify the government ' s responsibility , strengthen the strengthening , strengthen the supervision and management , promote the value - added of the insurance fund , strengthen the propaganda , promote the farmers , especially the old people , perfect the legal system , create the institutional environment , transform the pattern of rural economic development , and promote the virtuous circle of the rural economy .
