【摘要】：建立農(nóng)村醫(yī)療保障系統(tǒng),有助于國(guó)家政權(quán)向農(nóng)村深入、有利于解決農(nóng)村的醫(yī)療需求問(wèn)題。新型農(nóng)村合作醫(yī)療制度是由政府組織、引導(dǎo)和支持的農(nóng)民醫(yī)療互助共濟(jì)制度,研究和完善新型農(nóng)村合作醫(yī)療制度,具有重大戰(zhàn)略意義。本文選取了湖南省具有代表性的岳陽(yáng)地區(qū),分析湖南省推行新型農(nóng)村合作醫(yī)療制度所取得的成績(jī)和面臨的問(wèn)題,并結(jié)合所發(fā)現(xiàn)的問(wèn)題和不足制定出相應(yīng)的解決方案,旨在為湖南省新型農(nóng)村合作醫(yī)療制度進(jìn)行決策和完善時(shí)提供理論依據(jù)和參考意見(jiàn)。結(jié)果如下： 明確政府職責(zé)、強(qiáng)化健全管理機(jī)制。針對(duì)新型農(nóng)村合作醫(yī)療制度設(shè)計(jì)存在的弊端,指出政府應(yīng)建立和完善政府的責(zé)任機(jī)制,將農(nóng)村合作醫(yī)療的責(zé)任落到實(shí)處,確保各級(jí)政府做到真正意義上的高度重視,重點(diǎn)組織,用心實(shí)施,確保農(nóng)村合作醫(yī)療的成功。 新農(nóng)合醫(yī)療制度法制化。針對(duì)新型農(nóng)村合作醫(yī)療確�；痦槙尺\(yùn)行和消除參與主體的疑慮,要盡快實(shí)現(xiàn)合作醫(yī)療制度法制化,一方面能夠保障農(nóng)民籌資工作法制化、規(guī)范化,也贏取農(nóng)民群眾的信賴。 全力提升鄉(xiāng)村醫(yī)療隊(duì)伍的服務(wù)能力。針對(duì)地區(qū)間和行業(yè)內(nèi)醫(yī)療隊(duì)伍服務(wù)水平和行為的差異,指出須提高新型農(nóng)村合作醫(yī)療制度建設(shè)的經(jīng)費(fèi)投入,提高農(nóng)村醫(yī)院人才引進(jìn)和人才培養(yǎng)的資金投入,提高農(nóng)村醫(yī)院醫(yī)用設(shè)備的物質(zhì)投入。 加強(qiáng)建設(shè)面向貧困農(nóng)民的醫(yī)療救助制度。針對(duì)起付線和封頂上線不合理、補(bǔ)償比例偏低等情況,應(yīng)采取保大必須保小的補(bǔ)償機(jī)制,擴(kuò)大農(nóng)民的受益面,充分利用醫(yī)療基金。 因此,未來(lái)湖南省農(nóng)村合作醫(yī)療制度的發(fā)展要呈現(xiàn)以下特點(diǎn),即舉辦法制化、參與社會(huì)化、形式多樣化、管理規(guī)范化、城鄉(xiāng)一體化,并通過(guò)以下幾個(gè)方面推進(jìn)和完善農(nóng)村合作醫(yī)療制度：建立健全法制政府責(zé)任約束機(jī)制以明確地方各級(jí)政府責(zé)任、加強(qiáng)宣傳和引導(dǎo)以吸引農(nóng)民積極參與農(nóng)村合作醫(yī)療、調(diào)整和完善合作醫(yī)療個(gè)人籌資機(jī)制、建立合理的補(bǔ)償機(jī)制以完善合作醫(yī)療制度的基礎(chǔ)與效用。
[Abstract]:The establishment of rural medical security system will help the state power to go deeper into rural areas and solve the problem of rural medical needs. The new rural cooperative medical care system is organized, guided and supported by the government. It is of great strategic significance to study and perfect the new rural cooperative medical care system. This paper selects the representative Yueyang area of Hunan Province, analyzes the achievements and problems faced by Hunan Province in the implementation of the new rural cooperative medical system, and combines the problems and shortcomings found out to formulate a corresponding solution. The purpose of this paper is to provide theoretical basis and reference for the decision-making and perfection of the new rural cooperative medical system in Hunan Province. The results are as follows: Clear government responsibility, strengthen and perfect management mechanism. In view of the defects in the design of the new rural cooperative medical care system, it is pointed out that the government should establish and perfect the government's responsibility mechanism, carry out the responsibility of the rural cooperative medical care, and ensure that the governments at all levels attach great importance to it in a real sense, and focus on the organization. To ensure the success of rural cooperative medical system. New rural cooperative medical system legalization. In view of the new rural cooperative medical system to ensure the smooth operation of the fund and to remove the doubts of the main participants, it is necessary to realize the legalization of the cooperative medical system as soon as possible. On the one hand, it can guarantee the legalization and standardization of the peasants' financing work, and also win the trust of the peasants. All efforts will be made to improve the service capacity of rural medical teams. In view of the differences in service level and behavior of medical teams in different regions and industries, it is pointed out that it is necessary to increase the financial input for the construction of the new rural cooperative medical care system, and to increase the funds for the introduction and training of talents in rural hospitals. We will increase the material input of medical equipment in rural hospitals. We will strengthen the construction of a medical aid system for poor farmers. In view of the unreasonable starting line and top line, and the low proportion of compensation, we should adopt the compensation mechanism which must protect the small, enlarge the benefit area of farmers and make full use of medical fund. Therefore, the future development of the rural cooperative medical system in Hunan Province should show the following characteristics: organizing legalization, participating in socialization, diversification of forms, standardization of management, integration of urban and rural areas, And through the following aspects to promote and improve the rural cooperative medical system: establish and improve the legal system of government responsibility constraint mechanism to clarify the responsibilities of local governments, strengthen publicity and guidance to attract farmers to actively participate in rural cooperative medical care, Adjusting and perfecting the individual financing mechanism of cooperative medical system and establishing reasonable compensation mechanism to perfect the foundation and utility of cooperative medical system.
