【摘要】：目的了解學(xué)生聚集性結(jié)核病疫情發(fā)生的原因,為學(xué)校肺結(jié)核病防控提供依據(jù)。方法對嵊州市某學(xué)校出現(xiàn)病例所有班級學(xué)生進行結(jié)核菌素(PPD)試驗、X線胸部攝片,可疑者CT復(fù)查,對X線胸片檢查或CT復(fù)查異常者給予痰涂片檢查抗酸桿菌。結(jié)果首發(fā)病例為涂陽病人,患者出現(xiàn)癥狀后在密閉空調(diào)環(huán)境中與其他同學(xué)一起學(xué)習(xí)2個月,才被明確診斷并離校治療,增加交叉感染的發(fā)病概率。本起疫情通過因癥就診、轉(zhuǎn)診和篩查共發(fā)現(xiàn)肺結(jié)核病31例,均為學(xué)生病例。其中涂陽肺結(jié)核4例、涂陰肺結(jié)核25例、結(jié)核性胸膜炎2例;18例有臨床癥狀,13例無自覺癥狀;3-4月份為發(fā)病高峰。第1例陽性病例發(fā)現(xiàn)時間晚、就診延誤、持續(xù)接觸、通風(fēng)不良和學(xué)校分班是造成本次聚集性疫情的主要原因。結(jié)論應(yīng)加強學(xué)校對傳染病的管理,落實晨檢和癥狀監(jiān)測,以及時發(fā)現(xiàn)和隔離結(jié)核病病人,預(yù)防服藥是聚集性肺結(jié)核疫情中密切接觸者的有效預(yù)防手段。
[Abstract]:Objective to understand the causes of the epidemic of students' concentrated tuberculosis, and to provide the basis for the prevention and control of tuberculosis in schools. Methods the PPDs test was performed on all class students of a school in Shengzhou. X-ray chest radiographs, CT reexamination of suspicious cases, sputum smear examination of acid-fast bacilli were given to those with abnormal X-ray or CT reexamination. Results the first case was smear positive patients. Patients developed symptoms in the airtight air conditioning environment and other students to learn for 2 months before being clearly diagnosed and left school treatment to increase the incidence of cross-infection. A total of 31 cases of pulmonary tuberculosis were found by referral and screening, including 4 cases of smear positive pulmonary tuberculosis, 25 cases of smear negative pulmonary tuberculosis and 2 cases of tuberculous pleurisy. 18 cases had clinical symptoms and 13 cases had no conscious symptoms. March-April was the peak of the disease. The first positive case was found late, delayed visit and continued contact. Conclusion the management of infectious diseases in schools should be strengthened, morning checkup and symptom monitoring should be carried out in order to detect and isolate tuberculosis patients in time. Preventive medication is an effective means of prevention for close contacts in the epidemic of aggregated pulmonary tuberculosis.
