上海高校大學(xué)新生心理健康狀況及干預(yù)研究

【摘要】:【背景】心理疾患已成為嚴(yán)重的公共衛(wèi)生問題和較為突出的社會(huì)問題,大學(xué)生心理健康狀況與心理健康教育工作受到國(guó)家和社會(huì)的日益關(guān)注。目前我國(guó)大學(xué)生對(duì)心理疾患認(rèn)識(shí)不足;高校心理健康服務(wù)體系亟待完善。當(dāng)代精神衛(wèi)生服務(wù)對(duì)心理疾患的預(yù)防、早期識(shí)別與干預(yù)更受關(guān)注。提升高校心理健康服務(wù)能力對(duì)精神疾病的預(yù)防和治療起著重要作用。【目的】1.了解大學(xué)新生心理健康狀況,開展普遍性心理健康教育活動(dòng),并評(píng)估其效果。2.對(duì)在校大學(xué)生心理問題早期人群進(jìn)行重點(diǎn)干預(yù),建立兩種心理疾患(焦慮、抑郁/自殺)的規(guī)范化干預(yù)方案,并評(píng)估其效果。3.了解PHQ-9、GAD-7、PSS中文版在大學(xué)生人群中的信效度,確立大學(xué)生心理健康的評(píng)估工具!痉椒ā勘狙芯糠謩e采用橫斷面研究和對(duì)照研究方法。1.大學(xué)新生心理健康狀況及心理健康教育效果研究在上海交大新生入學(xué)時(shí)對(duì)其開展首次心理普測(cè)以了解基本心理健康狀況,然后開展普及性心理健康教育一年后進(jìn)行心理健康狀況前后對(duì)比。2.大學(xué)新生心理問題干預(yù)研究選擇上海交通大學(xué)新生進(jìn)行心理相關(guān)量表普測(cè)后,按入組標(biāo)準(zhǔn)確定入組對(duì)象,分為干預(yù)組和對(duì)照組,干預(yù)組接受干預(yù)后前后對(duì)比;同時(shí)與對(duì)照組進(jìn)行對(duì)比研究。3.PHQ-9、GAD-7、PSS中文版信效度測(cè)試PHQ-9、GAD-7量表測(cè)試對(duì)象為上海交通大學(xué)及華東師范大學(xué)四個(gè)年級(jí)學(xué)生,先集中進(jìn)行測(cè)試,計(jì)算量表的信度,其中以MINI(成人版)作為金標(biāo)準(zhǔn),分別研究其敏感度、特異度和ROC曲線系數(shù),并用SDS、SAS、SF-36分別作平行效度測(cè)試。完成首測(cè)二周左右,再在其中隨機(jī)選取10%進(jìn)行重測(cè)以了解重測(cè)信度。PSS量表測(cè)試對(duì)象為上海交大2011級(jí)學(xué)生。集中完成首測(cè)以計(jì)算信度系數(shù),同時(shí)使用PHQ-9、GAD-7做平行效度測(cè)試。完成首測(cè)一個(gè)月內(nèi)隨機(jī)抽取120人左右進(jìn)行重測(cè)以了解重測(cè)信度。【結(jié)果】1.大學(xué)新生心理健康狀況及心理健康教育活動(dòng)效果研究。首次測(cè)試結(jié)果PHQ-9均分為6.80±4.42;GAD-7均分為4.76±3.83;按分界值,無抑郁361人(34.45%),有抑郁687人(65.55%);無焦慮557人(53.15%),有焦慮491人(46.85%)。經(jīng)過宣教,學(xué)生抑郁評(píng)分下降、自測(cè)健康評(píng)分提高、自殺風(fēng)險(xiǎn)下降,精神衛(wèi)生與心理保健知識(shí)得分提高,變化具有顯著意義。2.大學(xué)新生心理問題干預(yù)研究。干預(yù)組經(jīng)干預(yù)后,抑郁、焦慮程度下降,較之對(duì)照組變化明顯且有顯著意義。3.量表的信效度研究。三個(gè)量表信度及重測(cè)信度值良好,平行效度及結(jié)構(gòu)效度值良好。【結(jié)論】1.以上海交大為代表的大學(xué)新生總體心理狀況基本尚可,但存在抑郁、自殺風(fēng)險(xiǎn)及應(yīng)激問題等。開展適合大學(xué)生人群的普遍性心理健康教育活動(dòng)是必要而有效的。2.針對(duì)大學(xué)生心理問題干預(yù),應(yīng)當(dāng)采用適合該人群特點(diǎn)的干預(yù)方式,如朋輩教育(咨詢)、團(tuán)隊(duì)治療等多種方法。研究證明此類方式是合適并有效的。3.在大學(xué)生人群建立一個(gè)完整、良好、持續(xù)的早期診斷、早期干預(yù)的心理健康管理及預(yù)防體系,有助于完善高校心理健康教育和管理工作。4.在大學(xué)生人群中,PHQ-9、GAD-7、PSS-10中文版具有良好的信度與效度。
