急診護理碩士專業(yè)學位研究生臨床實踐模式的研究

【摘要】:目的:本課題主要探索目前我國護理碩士專業(yè)學位研究生的研究方向設(shè)置情況,同時調(diào)查其臨床實踐情況,分析研究生臨床實踐中存在的一系列問題,在此基礎(chǔ)上,構(gòu)建出具有中國特色的急診護理碩士專業(yè)學位研究生臨床實踐模式,各高?蓞⒄沾四J綄痹\護理專業(yè)學位研究生進行統(tǒng)一培養(yǎng),以解決目前護理研究生教育水平參差不齊的問題,最終提高臨床護理服務(wù)質(zhì)量。對象:收集2016年招收護理碩士專業(yè)學位研究生的86所院校的招生信息,包括招生簡章、專業(yè)目錄以及導師研究方向,整理分析招生信息,總結(jié)目前我國設(shè)置的護理碩士專業(yè)學位研究生的研究方向;選取全國8所院校76名護理碩士專業(yè)學位研究生進行問卷調(diào)查,了解目前護理碩士專業(yè)學位研究生臨床實踐現(xiàn)況;根據(jù)專家的入選標準,綜合考慮專家代表性、研究領(lǐng)域多樣性等多種因素的存在,最終選取28名護理專家進行專家函詢。方法:了解我國護理碩士專業(yè)學位研究生的研究方向設(shè)置情況主要通過收集整理2016年招收護理碩士專業(yè)學位研究生的院校招生信息來實現(xiàn);了解我國護理碩士專業(yè)學位研究生臨床實踐情況及其存在的問題主要運用的研究方法是文獻分析法、半結(jié)構(gòu)式訪談法以及問卷調(diào)查法;運用文獻分析法、半結(jié)構(gòu)式訪談法,初步擬定急診護理碩士專業(yè)學位研究生臨床實踐模式的指標體系;通過德爾菲專家函詢法及統(tǒng)計學方法,對指標進行篩選與修改,最終確立急診護理碩士專業(yè)學位研究生臨床實踐模式。結(jié)果:1.2016年招收護理碩士專業(yè)學位研究生的86所高校中,20所高校明確表示不區(qū)分研究方向,10所高校的研究方向僅僅是護理臨床技能應(yīng)用與研究或臨床護理技能訓練與研究;在區(qū)分研究方向的56所高校中,其中10所的研究方向為護理教育、護理管理、臨床護理、社區(qū)護理等大概方向,其余46所將研究方向細化為外科護理、內(nèi)科護理、急危重癥護理等專科護理方向,這與我國國務(wù)院規(guī)定的護理碩士專業(yè)學位的培養(yǎng)目標基本一致。2.所有護理碩士專業(yè)學位研究生均參與臨床實習,且實習醫(yī)院均為三級甲等醫(yī)院。大部分院校臨床實踐輪轉(zhuǎn)科室為全科輪轉(zhuǎn),沒有根據(jù)研究生的研究方向選擇特色輪轉(zhuǎn)科室;關(guān)于研究方向,32.89%的學生選擇臨床護理,其中9人選擇急診急救護理方向;42.11%的研究生臨床實踐總時間為12個月以內(nèi),臨床實踐總時間偏少;80.26%的研究生所在院校沒有明確規(guī)定實踐內(nèi)容與要求;臨床帶教師資水平參差不齊,帶教方式與本科生沒有區(qū)別或區(qū)別不大,難以滿足研究生的學習需求;考核評價方式比較單一,沒有制定出切實可行的統(tǒng)一標準。3.兩輪專家咨詢的問卷有效回收率分別為90.32%和100%,可見,各位專家對本課題的研究持支持態(tài)度,并具有濃厚興趣;專家權(quán)威系數(shù)為0.879,表明各位專家是根據(jù)其理論知識和實踐經(jīng)驗來做出判斷的,并非憑空想象,可信度較高;肯德爾協(xié)調(diào)系數(shù)為0.314(c2=1370.602,p0.01),表明專家預測協(xié)調(diào)性比較好。根據(jù)指標篩選標準,同時結(jié)合專家意見對指標進行篩選及修改。本課題共經(jīng)過兩輪專家函詢,最后構(gòu)建的急診護理碩士專業(yè)學位研究生臨床實踐模式包含4個一級指標,17個二級指標和136個三級指標。結(jié)論:目前我國各院校根據(jù)國家教育部的總體要求制定適合本院校的專業(yè)型護理研究生臨床實踐模式,這導致各院校培養(yǎng)的研究生質(zhì)量參差不齊,嚴重影響我國護理教育事業(yè)的發(fā)展。因此,構(gòu)建科學、全面、規(guī)范的專業(yè)型護理研究生臨床實踐模式,有助于提升護理教育質(zhì)量,提高臨床護理水平,滿足醫(yī)院對高級護理人才的需求。本課題以臨床需求為導向,以崗位勝任力為核心制定出急診護理碩士專業(yè)學位研究生臨床實踐培養(yǎng)模式,以培養(yǎng)一批高素質(zhì)的急診?谱o理隊伍,以滿足當今社會對高水平急救護理的需求,同時為各高校培養(yǎng)和教育急診護理碩士專業(yè)學位研究生提供一定的參考依據(jù)。
[Abstract]:Objective: To explore the research direction of graduate students of nursing master's degree, investigate the clinical practice and analyze a series of problems in the clinical practice of graduate students, and on this basis, build a clinical practice model of postgraduate students with Chinese characteristics. Colleges and universities can refer to this model to unify the graduate students of emergency nursing degree, in order to solve the problems of the current level of nursing graduate education, and ultimately improve the quality of clinical nursing services. Object: to collect the enrollment information of 86 colleges and universities for postgraduate nursing master's degree in 2016, including the enrolment chapter and the professional list. At present, the main methods of studying the clinical practice of postgraduate nursing graduate students in China and their existing problems are literature analysis, semi structural interview and questionnaire survey, and using the method of literature analysis and semi structural interview to preliminarily draw up the indexes of the clinical practice model of the postgraduate students in emergency nursing master's degree. System; through the Delphy expert inquiry method and statistical method, the index is screened and modified, and the clinical practice model of the master degree graduate student of emergency nursing is established. Therefore, the construction of a scientific, comprehensive and standardized clinical practice model of professional nursing postgraduates is helpful to improve the quality of nursing education, improve the level of clinical nursing, and meet the high level of nursing care in the hospital. In order to meet the needs of the high level emergency nursing care in the present society, and to nurture and educate the emergency nursing care for all colleges and universities, this paper takes the post competency as the core to formulate the training mode of the postgraduate students' clinical practice in emergency nursing, in order to train a group of high quality emergency nursing teams to meet the needs of the high level emergency care in the present society. It provides some reference for Postgraduates of master's degree.
