【摘要】：目的醫(yī)學(xué)模式的轉(zhuǎn)變和社會(huì)經(jīng)濟(jì)的發(fā)展給醫(yī)學(xué)教育帶來了新的挑戰(zhàn)。本研究參考WFME認(rèn)可標(biāo)準(zhǔn),并運(yùn)用相關(guān)理論指導(dǎo),通過官網(wǎng)信息整理、文獻(xiàn)研究法、比較研究法和問卷調(diào)查法,分析我國(guó)臨床醫(yī)學(xué)專業(yè)認(rèn)證體系中存在的優(yōu)勢(shì)與不足,提出建議,為我國(guó)臨床醫(yī)學(xué)專業(yè)認(rèn)證體系的不斷發(fā)展完善提供參考。方法1.官網(wǎng)信息整理:通過瀏覽WFME官方網(wǎng)站,了解WFME認(rèn)可項(xiàng)目相關(guān)內(nèi)容,瀏覽CAAM-HP、TEPDAD、LCME及WCAME的官方網(wǎng)站,獲取相關(guān)信息并整理。2.文獻(xiàn)研究法:檢索中英文數(shù)據(jù)庫,包括中國(guó)知網(wǎng)(CNKI)、萬方數(shù)據(jù)庫、Pub Med和Web of Science,以認(rèn)證、醫(yī)學(xué)教育認(rèn)證、臨床醫(yī)學(xué)認(rèn)證、medical accreditation,accrediting agency,accrediting body,accreditation system為關(guān)鍵詞查閱認(rèn)證相關(guān)文獻(xiàn),了解研究進(jìn)展,為本研究的進(jìn)行提供支持。3.比較研究法:對(duì)通過WFME認(rèn)可的三個(gè)認(rèn)證機(jī)構(gòu)CAAM-HP、TEPDAD、LCME和我國(guó)臨床醫(yī)學(xué)專業(yè)認(rèn)證機(jī)構(gòu)WCAME進(jìn)行對(duì)比分析,并比較我國(guó)臨床醫(yī)學(xué)專業(yè)認(rèn)證工作委員會(huì)同WFME認(rèn)可標(biāo)準(zhǔn)之間的差異。4.問卷調(diào)查法:基于文獻(xiàn)研究和比較研究的結(jié)果以及我國(guó)臨床醫(yī)學(xué)專業(yè)認(rèn)證標(biāo)準(zhǔn)《中國(guó)本科醫(yī)學(xué)教育標(biāo)準(zhǔn)——臨床醫(yī)學(xué)專業(yè)(2016年)》制作問卷,對(duì)某省三所醫(yī)科大學(xué)的教師進(jìn)行問卷調(diào)查。5.統(tǒng)計(jì)分析:缺失值采用中位數(shù)進(jìn)行補(bǔ)齊。利用SPSS19.0和Stata13.0統(tǒng)計(jì)軟件進(jìn)行數(shù)據(jù)統(tǒng)計(jì)分析。對(duì)問卷進(jìn)行信效度分析,采用描述性統(tǒng)計(jì)分析方法,t檢驗(yàn)及回歸分析進(jìn)行統(tǒng)計(jì)分析。結(jié)果1.CAAM-HP、TEPDAD、LCME、WCAME的比較背景方面,CAAM-HP、TEPDAD、LCME為第三方機(jī)構(gòu),WCAME為教育部下設(shè)機(jī)構(gòu);認(rèn)證標(biāo)準(zhǔn)方面,四個(gè)機(jī)構(gòu)對(duì)認(rèn)證標(biāo)準(zhǔn)定期更新,所要求的領(lǐng)域基本相似,內(nèi)容存在差異,細(xì)化程度不同;程序和流程方面,四個(gè)機(jī)構(gòu)認(rèn)證流程比較統(tǒng)一,CAAM-HP、TEPDAD、LCME都十分看重認(rèn)證中學(xué)生的角色;政策和資源方面,CAAM-HP和LCME采取了簽署書面協(xié)議的方式避免利益沖突,WCAME采用了專家回避制度來避免利益沖突。2.WCAME與WFME認(rèn)可標(biāo)準(zhǔn)的比較目前我國(guó)臨床醫(yī)學(xué)認(rèn)證機(jī)構(gòu)為教育部設(shè)立,并不是第三方機(jī)構(gòu),沒有上級(jí)認(rèn)可機(jī)構(gòu)對(duì)其進(jìn)行認(rèn)可。認(rèn)證標(biāo)準(zhǔn)的制定基于對(duì)國(guó)際醫(yī)學(xué)教育標(biāo)準(zhǔn)的研究,進(jìn)行了本土化。認(rèn)證流程整體遵循WFME認(rèn)可標(biāo)準(zhǔn)的規(guī)范,投訴程序有待完善。WCAME制定了認(rèn)證專家的回避制度,建立了“臨床醫(yī)學(xué)專業(yè)認(rèn)證數(shù)據(jù)庫”,保存有關(guān)信息和資料。3.對(duì)某省三所醫(yī)科大學(xué)教師的問卷調(diào)查問卷Cronbach’sα系數(shù)為0.93,表明問卷信度較好。對(duì)問卷進(jìn)行探索性因子分析,結(jié)果表明問卷具有良好的結(jié)構(gòu)效度t檢驗(yàn)結(jié)果顯示正在準(zhǔn)備認(rèn)證的院校其教師對(duì)認(rèn)證相關(guān)內(nèi)容重要程度的評(píng)分更高。簡(jiǎn)單回歸分析結(jié)果顯示教師所在的學(xué)校、年齡、學(xué)歷、工作年限、職稱和從事工作類型影響教師對(duì)條目重要程度的評(píng)分。多元線性回歸結(jié)果顯示學(xué)校和從事工作類型是主要的影響因素。結(jié)論1.CAAM-HP、TEPDAD、LCME、WCAME四個(gè)機(jī)構(gòu)在背景、認(rèn)證標(biāo)準(zhǔn)、程序和流程、政策和資源四個(gè)方面擁有共性,同時(shí)也存在著一定的差異。2.WCAME基本符合WFME認(rèn)可標(biāo)準(zhǔn),一些方面仍有差異。3.認(rèn)證能夠促進(jìn)教師教育與管理理念的轉(zhuǎn)變。處于認(rèn)證周期不同階段的學(xué)校其教師對(duì)于認(rèn)證標(biāo)準(zhǔn)相關(guān)內(nèi)容重要程度的評(píng)分存在差異。同時(shí)教師的年齡、學(xué)歷、工作年限、職稱和從事工作類型也影響重要程度評(píng)分結(jié)果。
[Abstract]:The change of objective medical model and the development of social economy have brought new challenges to medical education. This study refers to the WFME accreditation standards, and uses the guidance of relevant theories, through the information arrangement of the official network, the literature research method, the comparative research method and the questionnaire survey method, and analyzes the advantages and disadvantages of the clinical medical professional certification system in China, and puts forward the advantages and disadvantages. Suggestions, provide reference for the continuous development and improvement of clinical medical professional certification system in China. Method 1. official website information sorting: through browsing the official website of WFME, understanding the relevant contents of WFME accreditation projects, browsing the official websites of CAAM-HP, TEPDAD, LCME and WCAME, obtaining relevant information and sorting out the.2. literature research method: retrieving Chinese and English databases, including the database of Chinese and English, including CNKI, Wanfang database, Pub Med and Web of Science, with authentication, medical education certification, clinical medical certification, medical accreditation, accrediting agency, accrediting body. The three accredited institutions approved by WFME, CAAM-HP, TEPDAD, LCME and WCAME of our country's clinical medicine professional certification body, are compared and analyzed, and the difference.4. questionnaire survey method is compared between the clinical medical professional certification committee and the WFME accreditation standard: the results based on the literature research and comparative study and the clinical medicine specialty of our country. The certification standard "Chinese undergraduate medical education standard - clinical medicine major (2016) > production questionnaire", three medical university teachers in a province, questionnaire survey.5. statistical analysis: the missing value used to make up the median, using SPSS19.0 and Stata13.0 statistical software to carry out data statistical analysis. Descriptive statistical analysis, t test and regression analysis. CAAM-HP, TEPDAD, LCME all attach great importance to the role of authenticated middle school students in both procedures and processes. In terms of policy and resources, CAAM-HP and LCME have signed a written agreement to avoid conflicts of interest, and WCAME adopts an expert avoidance system to avoid conflicts of interest.2.WCAME and WFM. E accreditation standards are now established by our clinical medical certification body for the Ministry of education. It is not a third party organization, which is not approved by the superior accreditation institution. The formulation of the certification standard is based on the research on international medical education standards. The certification process follows the standard of the WFME accreditation standard and the complaint procedure remains to be completed. Good.WCAME formulated a certification expert's avoidance system and established a "clinical medical professional certification database". The questionnaire survey questionnaire Cronbach 's alpha coefficient of three medical university teachers in a province was 0.93, which showed that the questionnaire was good. The questionnaire was analyzed by exploratory factor analysis. The results showed that the questionnaire was good. The results of the structural validity t test showed that the teachers who were preparing for the certification were higher on the importance of certification related content. The results of simple regression analysis showed that the teachers' schools, age, education, working years, titles and types of work affected the teachers' score on the importance of the items. The change of education and management concept. The school teachers at different stages of the certification cycle have different grades on the importance of the relevant content of the certification standards. At the same time, the age of the teachers, the education, the years of work, the title of the job and the type of work also affect the results of the importance.
